Before male breast reduction: Gynecomastia surgery: gland, chest fat and choosing an operation
Gynecomastia surgery, also called male breast reduction, treats gland, fat or both, with a separate plan for loose skin. Mayo Clinic distinguishes enlarged gland from chest fat alone. Learn what the examination, cause and grade mean for your operation, what can return and what your insurer requires. The cost and recovery guides explain quotes, drains, compression vests and returning to the gym.

Questions before gynecomastia surgery
Start with the diagnosis and cause, compare the operation proposed, then write down the tradeoffs, payment and follow-up plan. Each answer names its sources and ends with a question you can save for your surgeon.
- Gynecomastia surgery vs chest fat: how does a surgeon tell them apart?Gynecomastia surgery, also called male breast reduction, can remove gland and fat; your surgeon examines you to tell them apart.
- What do gynecomastia grades mean before gynecomastia surgery?A grade describes your chest before gynecomastia surgery; Simon grades describe enlargement and skin, while Rohrich grades also consider tissue weight.
- Which causes and medicines need checking before gynecomastia surgery? Before gynecomastia surgery, your clinician checks medicines and medical conditions that could explain your enlargement.
How do steroids, prohormones and TRT affect gynecomastia surgery? Before gynecomastia surgery, tell your surgeon about steroids, prohormones and prescribed TRT, as ASPS recommends; they can affect enlargement and results.
What medical tests do I need before gynecomastia surgery? Before gynecomastia surgery, your clinician starts with your history and an examination, then chooses tests based on the findings.- When does a doctor check for breast cancer before gynecomastia surgery? Before gynecomastia surgery, contact your doctor about a firm or hard lump, one-sided enlargement, nipple discharge or skin dimpling.
Who fits gynecomastia surgery, and who should wait?Gynecomastia surgery may fit healthy adults with persistent enlargement; SIAMS’s corrected 2026 guideline recommends waiting at least 6 months after secondary causes resolve.- Which gynecomastia surgery technique fits gland, fat or both?Gynecomastia surgery uses liposuction, excision or both, depending on your tissue, says the American Society of Plastic Surgeons (ASPS).
- Where do the incisions go in gynecomastia surgery?Gynecomastia surgery incisions depend on the tissue and skin being treated; ASPS describes small liposuction openings and incisions for excision.
When does gynecomastia surgery need skin removal or nipple grafts?Gynecomastia surgery may need skin removal or nipple repositioning when skin is stretched or the chest droops markedly.- Can gynecomastia surgery treat only one side?Gynecomastia surgery can treat one side; ASPS says enlargement may affect one breast or both.
Is gynecomastia surgery done under local or general anesthesia? At University of Utah Health, gynecomastia surgery takes about 1–3 hours under outpatient general anesthesia; other hospitals describe local anesthesia or sedation.- What risks should I weigh before gynecomastia surgery?Gynecomastia surgery risks include bleeding, fluid collections, healing problems, uneven contours, permanent scars and lasting sensation changes; ASPS gives no percentages.
- Can revision gynecomastia surgery fix remaining gland or an uneven result?Revision gynecomastia surgery may treat remaining gland, loose skin or uneven contours; ASPS says results settle over 3–6 months.
Will gynecomastia come back after gynecomastia surgery?Gynecomastia can come back after gynecomastia surgery; ASPS warns that causative medicines, steroids or weight gain can affect lasting results.- Can fat freezing, Kybella or tamoxifen replace gynecomastia surgery?Fat freezing and Kybella aren’t FDA-approved for male breasts, and tamoxifen isn’t approved specifically for gynecomastia; ask before replacing gynecomastia surgery.
- Does insurance cover gynecomastia surgery under my plan?Coverage for gynecomastia surgery depends on your member benefit contract and your insurer’s policy; pain or preauthorization alone doesn’t promise payment.
- What do Utah insurance policies say about gynecomastia surgery? For gynecomastia surgery, Regence’s September 1, 2026 policy requires enlargement lasting at least 12 months at age 19 or older.
- What does Medicare require for gynecomastia surgery in my state?Medicare’s gynecomastia surgery rules vary locally; Utah’s Noridian policy names grade III/IV or abnormal development with extra tissue.
- Who performs gynecomastia surgery, and how do I compare different plans? Before gynecomastia surgery, check board certification; ABPS requires at least 6 years of surgical residency, including at least 3 in plastic surgery.
What should I ask before committing to gynecomastia surgery? Before gynecomastia surgery, ask for your diagnosis, exact operation, tradeoffs, follow-up and full quote, as the Aesthetic Society recommends.
Start with the diagnosis
Gynecomastia surgery vs chest fat: how does a surgeon tell them apart?
Gynecomastia surgery, also called male breast reduction, can remove gland and fat; your surgeon examines you to tell them apart. Mayo Clinic’s October 2023 definition separates true gynecomastia, enlarged gland, from pseudogynecomastia, extra fat without enlarged gland. A photograph or nipple squeeze can’t diagnose either.

Gland and fat need different treatment plans
True gynecomastia
- What the source describes
- Mayo Clinic’s October 26, 2023 definition is enlarged gland tissue. Extra fat can be present too
- What an examination can show
- Cleveland Clinic’s July 9, 2024 page describes a movable, sometimes tender growth beneath the nipple
- What treatment targets
- Mayo distinguishes gland removal from liposuction of fat
Pseudogynecomastia
- What the source describes
- Mayo’s definition is increased fat without increased gland
- What an examination can show
- Cleveland Clinic describes fatty fullness spread across the chest. You still need an examination
- What treatment targets
- The Aesthetic Society describes weight loss or liposuction for fat-dominant enlargement
An uncertain lump
- What the source describes
- A lump isn’t a home diagnosis
- What an examination can show
- Mayo describes your history, examination of the breasts, abdomen and genitals, and tests chosen for your findings
- What treatment targets
- An uncertain or suspicious lump needs diagnosis before you plan cosmetic surgery
As listed on October 13, 2026, these sources explain tissue types and examinations without saying how accurate each check is.
Cleveland Clinic’s July 2024 page describes gland as a movable, sometimes tender growth beneath the nipple. Fatty enlargement is more spread out. The 2012 American Family Physician review describes firm tissue centered around the nipple and areola. Mayo describes examining the breasts, abdomen and genitals, with tests chosen for your findings. Cleveland Clinic says weight loss may reduce fat, but exercise is unlikely to reverse hormonally enlarged gland. 22 of 70 patient accounts we read discussed exercise or weight loss not resolving their chest concern.
“Do I have gland, fat or extra skin, and what would surgery change?”
Start with the diagnosis
What do gynecomastia grades mean before gynecomastia surgery?
A grade describes your chest before gynecomastia surgery; Simon grades describe enlargement and skin, while Rohrich grades also consider tissue weight. The Baylor/Methodist authors’ May 2015 review shows each system separately. Rohrich also considers what the tissue consists of and how much the chest droops.
Simon grades describe size and skin in the 2015 review
Rohrich grades also consider tissue weight and drooping
The review uses Simon’s 1973 grades and Rohrich’s 2003 system without adding new measurements. Rohrich’s under-250 g and 250–500 g categories estimate breast tissue, not body weight or something you can measure at home. Waltho and colleagues’ 2017 review found 11 systems across 11 included articles, each using different features. You can’t swap one system’s grade for another’s. Insurers may use their own ASPS-adapted grades. A grade alone doesn’t tell you which operation you need or whether insurance will pay.
“Which grading system are you using, and how do my skin, tissue and nipple position affect the plan?”
Find and address the cause
Which causes and medicines need checking before gynecomastia surgery?
Before gynecomastia surgery, your clinician checks medicines and medical conditions that could explain your enlargement. Mayo Clinic’s October 2023 page lists both. A medicine on that list may not be your cause, and treating a cause may change whether you need surgery.
Medicines and conditions to discuss with your clinician
Mayo Clinic’s 2023 page lists antiandrogens, finasteride, spironolactone, certain HIV medicines and some psychiatric or heart medicines.
Mayo lists hypogonadism, hormone-producing tumors, hyperthyroidism, kidney failure and liver disease.
Nuttall and colleagues’ 2015 review covered 110 publications, including 63 single-case reports. Enlargement after starting a drug doesn’t prove the drug caused it.
EAA 2019 and SIAMS 2026 say a suspected medicine cause shouldn’t stop checks for another disease.

Mayo lists antiandrogens, finasteride, spironolactone, certain HIV medicines and some psychiatric or heart medicines. It also lists low testosterone, tumors, thyroid, kidney and liver disease. Nuttall and colleagues’ 2015 review covered 110 publications, including 63 single-case reports. They cautioned that it can be hard to tell whether a medicine caused someone’s enlargement.
EAA 2019 and SIAMS 2026 say a suspected medicine cause shouldn’t stop checks for another disease. Cleveland Clinic says any prescription change belongs with its prescriber.
Endotext’s 2023 chapter describes improvement within a month after removing a causative medicine. AFP’s 2012 review says enlargement often shrinks within 3 months. Neither says how many people improved, and long-standing hardened gland may remain.

How do steroids, prohormones and TRT affect gynecomastia surgery?
Before gynecomastia surgery, tell your surgeon about steroids, prohormones and prescribed TRT, as ASPS recommends; they can affect enlargement and results. The July 2025 AndroGel 1% label warns that gynecomastia may develop and persist during androgen treatment for low testosterone.
Steroids, prohormones and TRT can affect your chest differently
More from the sources
Endotext’s January 2023 chapter says testosterone often doesn’t reverse established gland and calls testosterone-induced enlargement uncommon and transient. The product label covers a different question; its flexible-dose extension reports 2.5% among 162 treated subjects over up to 3 additional years. That describes side effects during drug treatment, rather than recurrence after surgery.
Endotext lists androstenedione and DHEA as substances the body can turn into estrogen. FDA’s September 2024 warning says some bodybuilding supplements contain steroids or steroid-like substances. It warns about dangerous withdrawal after suddenly stopping these products.
Bring names, ingredients and planned use to your clinician. These sources don’t give a reliable percentage for recurrence after surgery while using steroids or TRT.
“How will you and my prescriber coordinate my TRT, steroid or prohormone plan before and after surgery?”
Find and address the cause
What medical tests do I need before gynecomastia surgery?
Before gynecomastia surgery, your clinician starts with your history and an examination, then chooses tests based on the findings. AFP’s 2012 review, Endotext’s 2023 chapter and SIAMS’s corrected 2026 guideline recommend different blood tests. Their lists don’t add up to one panel everyone needs.
Your findings determine which blood tests you need
Testicular ultrasound is a separate decision
More from the sources
AFP recommends liver, kidney and thyroid tests first, while Endotext starts with testosterone, estradiol, LH and hCG. SIAMS recommends choosing hormone tests for your findings. It cautions about estradiol testing and cutoffs that haven’t been validated. Its recommendation and table also place estradiol differently, and recommendations for testicular ultrasound differ too.
AFP recommends it for selected findings, while Endotext uses suspected masses or hormone findings. EAA 2019 and SIAMS 2026 recommend ultrasound more broadly alongside the genital examination.
These guidelines don’t say how many surgical patients had a cause found through testing. Insurers’ laboratory rules decide benefits separately. Breast imaging alone doesn’t check every possible cause.

When does a doctor check for breast cancer before gynecomastia surgery?
Before gynecomastia surgery, contact your doctor about a firm or hard lump, one-sided enlargement, nipple discharge or skin dimpling. Mayo Clinic’s October 2023 page flags these findings for assessment. ACR’s 2018 guidance uses age 25 to choose imaging for an uncertain mass. Suspicious findings need diagnostic imaging at any age.
The examination guides whether you need breast imaging
More from the sources
Contact your doctor about these findings before booking elective chest surgery. ACR says routine initial breast imaging is usually not appropriate when the examination clearly shows gland or fat enlargement. An uncertain or suspicious mass needs a different approach. EAA 2019 suggests core needle biopsy for suspicious findings, while SIAMS 2026 specifies mammography before biopsy in that situation.
A 2024 US report found ductal carcinoma in situ in both breasts of one 47-year-old man after major weight loss. It was found in tissue removed during surgery; the authors argue for sending removed tissue to pathology. That single case shows this can happen but gives no routine cancer rate. Ask who examines your removed tissue and who receives the result.
“Do I need a cancer evaluation first, and will my removed gland go to pathology?”
Decide whether to proceed
Who fits gynecomastia surgery, and who should wait?
Gynecomastia surgery may fit healthy adults with persistent enlargement; SIAMS’s corrected 2026 guideline recommends waiting at least 6 months after secondary causes resolve. ASPS also describes stable breast development, realistic goals and consideration of medical alternatives. The SIAMS wait applies before considering medicine or surgery.
What to check before deciding when to have surgery
ASPS describes healthy adults with relatively normal weight, stable breast development and realistic goals. Medical alternatives haven’t corrected their enlargement.
SIAMS 2026 recommends watching for at least 6 months after secondary causes resolve before considering medicine or surgery. That wait doesn’t tell you how often enlargement shrinks or what your insurer requires.
ASPS describes candidates as nonsmokers. The Aesthetic Society says skin may not tighten well after major weight loss. There’s no universal adult BMI cutoff in these sources.
Mayo’s 2023 page says pubertal enlargement often resolves within 2 years. Pediatric Endocrine Society’s June 23, 2021 guidance waits for growth and puberty completion, without one universal birthday threshold.
SIAMS 2026 suggests distress screening and selected counseling before and after treatment. Taking time to clarify the change you want belongs in the consultation.
More from the sources
That wait is clinical guidance. It doesn’t tell you how often enlargement shrinks or how long your insurer requires you to wait. ASPS describes relatively normal weight and nonsmoking, without a universal BMI cutoff. The Aesthetic Society says poor skin elasticity affects tightening after major weight loss.
Mayo’s 2023 page says enlargement during puberty often resolves within 2 years. The Pediatric Endocrine Society’s June 2021 guidance waits for growth and puberty completion, without one birthday cutoff. Endotext says delaying surgery during puberty reduces concern about recurrence. ASPS notes that continuing development can lead to another operation. Those are puberty facts only. This guide is for adults age 18 and older.
SIAMS also suggests psychological support for some people. You can take more time to decide.

Choose the operation
Which gynecomastia surgery technique fits gland, fat or both?
Gynecomastia surgery uses liposuction, excision or both, depending on your tissue, says the American Society of Plastic Surgeons (ASPS). Liposuction removes fat; excision removes gland or extra skin. A small incision or a tool name such as PAL or VASER doesn’t tell you which tissues will be treated.
What each operation targets
Targets fatty tissue. A gland that remains may need direct excision.
Targets gland through an incision. The drawing does not imply all tissue under the nipple is removed.
Addresses fat and gland as separate tissue components, as ASPS describes.
Adds skin removal when needed. Free grafting is one described approach, with possible sensation and pigment changes.
Blue marks the tissue target on the same base; it does not show how much will be removed. A schematic adult chest, not a diagnosis, measurement, surgical plan or predicted result. Incision sites and tissue removal differ by operation.
Match the operation to the tissue
Liposuction alone
- When it’s used
- Mainly fat, without very loose skin
- What it does
- ASPS and Stanford describe suctioning fat through small openings for a thin tube
- What to clarify
- Dense gland can remain. Ask separately about removing skin or moving the nipple
Gland excision
- When it’s used
- Gland that needs direct removal
- What it does
- ASPS describes cutting out gland. The Baylor/Methodist review describes an incision at the lower edge of the areola
- What to clarify
- That review recommends leaving enough tissue beneath the areola to avoid a hollow. It doesn’t say how often this prevents one
Liposuction with excision
- When it’s used
- Fat plus gland
- What it does
- ASPS describes using both methods in the same plan
- What to clarify
- Ask which tissue each part removes and what skin is expected to do afterward
Pull-through excision
- When it’s used
- Fat and fibrous tissue when your surgeon recommends this approach
- What it does
- Hammond and colleagues’ 2003 Michigan description combines ultrasonic liposuction with direct tissue removal through a 1-cm incision
- What to clarify
- The authors don’t say how many adults had each outcome. A small opening still leaves an incision
ASPS and Stanford pages, as listed on October 13, 2026, with Baylor/Methodist 2015 and Hammond’s Michigan 2003 technique description. They don’t compare success rates for adults.
Liposuction names describe tools
Suction-assisted
- How the tool works
- ASPS describes a hollow tube connected to vacuum to remove fat
- Limit
- Removing fat doesn’t mean dense gland is removed too
Power-assisted, PAL
- How the tool works
- Mayo’s September 19, 2024 general liposuction page describes a tube moving rapidly back and forth
- Limit
- The page doesn’t say this works better for gynecomastia
Ultrasound-assisted, including VASER
- How the tool works
- ASPS describes ultrasound energy to liquefy fat; the 2015 Baylor/Methodist review identifies VASER as an ultrasound-assisted method
- Limit
- That review cautions about heat injury. These sources don’t give a reliable controlled comparison showing which tool works better for adults
A tool name doesn’t replace the plan for gland, skin and nipple position.

Tissue and areola access
Stanford describes liposuction for mild-to-moderate fatty enlargement when the skin isn’t loose, and excision for gland or loose skin. The Baylor/Methodist 2015 review describes access at the lower edge of the areola. It recommends leaving tissue beneath the nipple to avoid a hollow, but doesn’t say how often that prevents one.
Pull-through excision
Hammond and colleagues’ 2003 Michigan description combines ultrasonic liposuction with pull-through excision through one 1-cm incision. It doesn’t say how many adults had each outcome.
Liposuction tools and patient questions
Mayo’s September 2024 liposuction page describes the rapidly moving tube used in power-assisted liposuction. The 2015 review describes VASER as ultrasound-assisted and cautions about burns from heat. 16 of 70 patient accounts we read discussed liposuction, gland removal or both.
“What removes my fat, what removes my gland, and what would liposuction alone leave behind?”

Where do the incisions go in gynecomastia surgery?
Gynecomastia surgery incisions depend on the tissue and skin being treated; ASPS describes small liposuction openings and incisions for excision. The Baylor/Methodist 2015 review describes access along the lower areolar edge, with longer chest-fold scars for extreme skin excess and free nipple grafting.
Where the access patterns differ
Small entry points let a cannula reach fatty tissue. Locations vary with the technique and surgical plan.
An incision along the lower areolar edge gives access to gland. The Baylor/Methodist review describes retaining tissue beneath the areola to avoid a depression.
The Michigan authors describe direct tissue removal through the small liposuction access incision. This is a technique description, with no comparative outcome claim.
For marked drooping and extreme skin excess, the Baylor/Methodist review describes removal along the chest fold and replacement of the nipple-areola as grafts. Sensation and pigment can change.
Alternative patterns on the same adult chest. These are not four steps of one operation. A schematic adult chest, not a diagnosis, measurement, surgical plan or predicted result. Incision sites and tissue removal differ by operation.
Ask your surgeon to mark the access pattern
Small openings
ASPS describes liposuction through small openings. Hammond’s 2003 Michigan pull-through description uses a 1-cm access incision for direct tissue removal.
The areola edge
The Baylor/Methodist 2015 review describes gland access along the lower areolar edge and retaining tissue beneath the nipple to avoid a hollow.
Longer skin-removal scars
The same review describes chest-fold removal with free nipple grafts for extreme skin excess. Ask how far your scars would extend and what happens to nipple feeling and pigment.
Alternative access patterns, not steps of one operation. ASPS and the 2003/2015 descriptions don’t compare adult success rates.
Hammond and colleagues’ 2003 Michigan pull-through description uses a 1-cm incision for ultrasonic liposuction and direct tissue removal. Incision size doesn’t tell you what tissue is removed or promise the final contour. Ask your surgeon to mark every proposed opening and explain the permanent scars, nipple position and sensation tradeoffs.
“Can you mark each incision on my chest and explain what it allows you to remove?”
Choose the operation
When does gynecomastia surgery need skin removal or nipple grafts?
Gynecomastia surgery may need skin removal or nipple repositioning when skin is stretched or the chest droops markedly. ASPS and the Baylor/Methodist 2015 review describe these options. A free nipple graft for extreme skin excess comes with possible loss of feeling and pigment.
Skin removal and free nipple grafts
For marked drooping and extreme skin excess, the Baylor/Methodist review describes removal along the chest fold and replacement of the nipple-areola as grafts. Sensation and pigment can change.
Skin and nipple position require their own plan
Leave skin in place
- When described
- When the chosen tissue-removal plan doesn’t directly remove skin
- What changes
- Hammond’s 2009 review describes skin removal initially or later as needed
- Tradeoff
- Skin may not tighten well if it has poor elasticity. A smaller incision doesn’t mean loose skin or low nipples will correct themselves
Keep the nipple attached during tissue removal
- When described
- Gland or skin removal with the nipple kept attached to tissue
- What changes
- The 2015 Baylor/Methodist review describes retaining its blood supply
- Tradeoff
- That review says some excess tissue may remain; ask about the scar pattern and nipple position
Skin excision with free nipple grafts
- When described
- Marked drooping and extreme skin excess, including after major weight loss
- What changes
- The same review describes removal along the chest fold and replacing the nipple-areola as grafts
- Tradeoff
- Possible loss of nipple sensation and pigment; larger permanent scars. A graft isn’t a promise of preserved feeling
These 2009 and 2015 descriptions don’t compare success rates. A 2025 nerve-reconnection report involved one 32-year-old man, assessed at 18 months. It doesn’t tell you what feeling to expect after routine grafting.
More from the sources
Skin elasticity and timing
The Aesthetic Society says poor skin elasticity changes how well skin tightens, including after gastric bypass or major stretch marks. Hammond’s 2009 review describes skin removal during the first operation or later if needed.
Attached nipple or graft
The 2015 review describes keeping the nipple attached to tissue, or removing and replacing it as a graft along a chest-fold incision. Neither plan fits everyone.
Limits of a nerve report
A 2025 US nerve-reconnection report involved one 32-year-old man assessed at 18 months. He still couldn’t distinguish two points touching the nipple and areola. That report doesn’t predict feeling after routine grafting. Ask separately about tissue removal, skin removal and nipple height, including the permanent scars you would accept.
“Will you remove skin or lift my nipples, and what scars or changes in feeling should I expect?”
Choose the operation
Can gynecomastia surgery treat only one side?
Gynecomastia surgery can treat one side; ASPS says enlargement may affect one breast or both. Mayo’s October 2023 page says one-sided enlargement or a firm lump needs medical evaluation before assuming it’s a cosmetic concern.
Examine both sides; plan each side
- The enlarged sideMayo calls for medical assessment of one-sided enlargement or a firm lump before assuming it is cosmetic.
- The other sideASPS describes enlargement on one side or both and lists asymmetry as a surgical risk. Ask about the plan for each side.
A schematic adult chest, not a diagnosis, measurement, surgical plan or predicted result. Incision sites and tissue removal differ by operation.
Both sides need examination, even for a one-sided operation
The enlarged side
ASPS says gynecomastia can affect one breast. Mayo’s 2023 page says one-sided enlargement or a firm lump needs medical assessment.
What did the examination show, and is cosmetic surgery appropriate?
The other side
ASPS lists uneven sides as a surgical risk. Record how the sides differ before surgery; an exact match can’t be promised.
Would you treat my other side too, and why?
ASPS and Mayo guidance, as listed on October 13, 2026. One-sided treatment came up in 1 of 70 patient-voice sources.
1 of 70 patient accounts we read discussed one-sided treatment. The clinical sources don’t compare complication rates for one-sided and two-sided operations or promise symmetry. Ask your surgeon to record the starting difference and explain what the examination found. Ask whether they propose treating the other side, and why. One patient’s office procedure doesn’t tell you whether local anesthesia or that technique fits your case. If you have a new one-sided lump, contact your doctor before elective surgery.
“Why do you recommend treating one side or both, and how close a match can I expect?”

Is gynecomastia surgery done under local or general anesthesia?
At University of Utah Health, gynecomastia surgery takes about 1–3 hours under outpatient general anesthesia; other hospitals describe local anesthesia or sedation. That timing covers Utah’s gynecomastia options. University of Michigan Health describes general anesthesia, with local anesthesia plus IV sedation possible for some patients.
Your operation helps determine the anesthesia plan
More from the sources
These undated hospital pages were checked October 13, 2026. They give expected times without saying how many patients were measured. MedStar describes local anesthesia, sedation or general anesthesia, while ASPS lists IV sedation or general anesthesia chosen by your physician. None says an awake procedure is safer or better for your operation.
ASPS describes accredited offices, licensed surgery centers or hospitals. It asks you to arrange a driver and an adult helper for at least the first night. Ask who gives anesthesia, how you’re monitored and why it fits the gland and skin work planned.
“Why does this anesthesia fit my operation, who gives it, and is the facility licensed or accredited?”
Understand the tradeoffs
What risks should I weigh before gynecomastia surgery?
Gynecomastia surgery risks include bleeding, fluid collections, healing problems, uneven contours, permanent scars and lasting sensation changes; ASPS gives no percentages. Innocenti and colleagues’ 2022 review covered 94 articles and 7,294 patients, including children and patients outside the US. It counts complications rather than necessarily counting each affected person, so it can’t tell you your own risk.
Risks ASPS names without a percentage
- Hematoma, a collection of blood
- Seroma, a collection of fluid
- Infection and poor wound healing
- Uneven sides or contours
- Permanent scars
- Potentially permanent sensation changes
- Another operation may be needed
These reviews can’t tell you your own complication risk
More from the sources
Holzmer and colleagues’ 2020 review included 17 studies and 1,112 patients. Its blood- and fluid-collection calculations used a separate group of 906 patients. The groups compared for drain use had different proportions of grade III cases. That makes it hard to tell whether drains affected the complications. Neither review reliably ranks operations for you. Ask about your health, the amount of tissue involved and your surgeon’s documented experience.
Get immediate medical attention for shortness of breath, chest pain or an unusual heartbeat after surgery, as ASPS advises. Call your surgeon right away about concerning swelling, bleeding, redness, drainage or pain. University of Utah Health tells patients to call the doctor for abnormal swelling, dressing drainage, redness or fever. The sources don’t give one universal fever cutoff.

Understand the tradeoffs
Can revision gynecomastia surgery fix remaining gland or an uneven result?
Revision gynecomastia surgery may treat remaining gland, loose skin or uneven contours; ASPS says results settle over 3–6 months. The Baylor/Methodist 2015 review separates these problems from gland growing again. Another operation may be needed, but the sources give no universal safe revision date.
Remaining gland, a hollow and loose skin need different repair plans
More from the sources
A hollow after tissue removal needs a different assessment from fullness caused by gland left behind. The 2015 review describes direct excision for remaining gland and leaving enough tissue beneath the areola to avoid a hollow. It doesn’t say how often repair succeeds. Utah Health offers revision consultations and says severity and other factors affect how flat the chest becomes.
Innocenti’s 2022 review groups revision and recurrence together. It can’t give you a separate chance of revision or gland regrowth. Call the surgeon right away for concerning new swelling or bleeding. Ask for the original operation report, what needs repair, what repair can achieve and each provider’s charges.
“What would revision treat, when could we assess it safely, and which surgeon, facility and anesthesia charges would I pay?”
Understand the tradeoffs
Will gynecomastia come back after gynecomastia surgery?
Gynecomastia can come back after gynecomastia surgery; ASPS warns that causative medicines, steroids or weight gain can affect lasting results. These sources don’t give a reliable percentage for recurrence after surgery with steroid or TRT use. New fullness can also be tissue left behind, swelling or scar changes.
New fullness doesn’t have one explanation
More from the sources
Blau and Hazani’s 2015 private-practice series reported no recurrence among 1,073 bodybuilders followed for 1–5 years out of 1,574 treated from 1980–2013. Some patients weren’t followed, and the authors didn’t analyze recurrence separately by steroid or other exposure. It can’t show that recurrence is impossible or that continuing steroids is safe.
The Aesthetic Society warns that causative medicines or anabolic steroids can cause recurrence. The Baylor/Methodist 2015 review separates remaining gland from gland regrowth. It says new growth raises concern about an underlying condition affecting the body. Ask your clinician to examine new fullness, and contact your surgeon right away if swelling, pain or bleeding is concerning.
“How will you check whether new fullness is swelling, scar tissue, remaining gland or regrowth, given my medicine history?”

Can fat freezing, Kybella or tamoxifen replace gynecomastia surgery?
Fat freezing and Kybella aren’t FDA-approved for male breasts, and tamoxifen isn’t approved specifically for gynecomastia; ask before replacing gynecomastia surgery. Mayo Clinic’s October 2023 page describes the medicine restriction. These treatments haven’t been shown to match surgery for long-standing gland.
What other treatments can do, and where approval stops
More from the sources
FDA’s October 2025 page describes fat freezing for fat bulges, without clearance or approval for breasts. Its September 2026 Kybella warning limits approval to fullness beneath the chin in adults and advises against other areas.
Endotext 2023 considers medicine for selected recent, tender enlargement with no known cause. EAA 2019 discourages general SERM use but discusses an exception for recent, painful enlargement. SIAMS 2026 suggests a short tamoxifen course for selected patients as expert opinion and off-label use.
A 1990 Colorado crossover study found pain improvement with tamoxifen in 5 of 6 men versus 1 of 6 with placebo. Each treatment lasted 2–4 months; participant ages weren’t given in the abstract. These six men can’t tell you how broadly the medicine helps or whether it can replace surgery for shape. Discuss medication decisions with your clinician.

Does insurance cover gynecomastia surgery under my plan?
Coverage for gynecomastia surgery depends on your member benefit contract and your insurer’s policy; pain or preauthorization alone doesn’t promise payment. Cigna 0195, effective August 15, 2026, offers different ways to meet medical-necessity rules. Aetna CPB 0017, reviewed March 12, 2026, calls these procedures cosmetic. A grade alone doesn’t promise payment either.

Aetna CPB 0017
Last reviewed March 12, 2026
- Whose plan
- Aetna plans. Your member contract decides your benefits.
- Ways to qualify and what’s required
- Aetna calls reduction or mastectomy on either or both breasts, and liposuction for gynecomastia, cosmetic. Its coverage rules for enlarged female breasts concern a different diagnosis.
- What to check
- Aetna says this bulletin guides benefit decisions without replacing your contract, and pain alone doesn’t meet its medical-necessity rules.

Cigna 0195
Effective August 15, 2026
- Whose plan
- Cigna plans. Your member benefits take priority over this policy.
- Ways to qualify and what’s required
- You need either Klinefelter syndrome OR every requirement in the duration route. That route requires enlargement starting during puberty for at least 2 years, or after puberty for 1 year. Gland must be found on examination and/or mammography, and you need ASPS grade II–IV with front and side photos, and persistent pain despite pain medicine. Causative substances must be stopped, when medically appropriate, for at least 1 year. Enlargement must remain after its cause is corrected. Hormonal causes must be checked, and any identified cause treated for at least 12 months.
- What to check
- The 12-month hormonal-treatment rule applies to the duration route, not the Klinefelter alternative. Liposuction alone doesn’t qualify as medically necessary. Appearance or emotional and social concerns alone don’t qualify either.

UnitedHealthcare MP.012.22
Effective June 1, 2026
- Whose plan
- Commercial and Individual Exchange plans only.
- Ways to qualify and what’s required
- For age 18 and older, ALL adult criteria must be met. These include stage II–IV with moderate or severe pain that limits function or physical ability. Gland must be the main tissue. Enlargement must persist after suspected causative drugs are stopped, with substance screening completed. Medical causes need appropriate evaluation and normal supporting laboratory tests.
- What to check
- UnitedHealthcare says most benefit plans exclude benign gynecomastia treatment. Sports or social limitations don’t qualify as functional impairment under this policy. Its 2-year rule applies to members under 18, rather than adults.
Policy text is not a coverage guarantee.
Anthem/Elevance CG-SURG-88
Published April 15, 2026; reviewed February 19, 2026
- Whose plan
- Plans using this guideline. Reconstructive benefits vary.
- Ways to qualify and what’s required
- The routine route requires ALL criteria. You must be over age 18 or at least 18 months past puberty. You need gland, diagnostic testing and a cause unrelated to obesity, puberty or anything reversible. Pain must affect daily activities and persist despite 3 months of pain medicine or anti-inflammatory treatment. Photos are required. Drug-induced enlargement must remain 6 months after causative therapy stops. A legitimate cancer concern is a separate route. A separate reconstructive route applies when medical-necessity criteria aren’t met. It uses the same age/puberty threshold and requires drug-induced enlargement to persist 6 months after therapy stops.
- What to check
- Cancer concern, routine medical necessity and reconstructive benefits have different rules. The guideline doesn’t consider liposuction used to perform the mastectomy medically necessary.

BCBS Michigan/BCN
Surgical Treatment for Male Gynecomastia; effective January 1, 2026; no numeric policy id shown
- Whose plan
- BCBSM/BCN member plans in Michigan.
- Ways to qualify and what’s required
- You need gland measuring at least 2 cm AND one of two findings. Enlargement starting during puberty must last over 2 years, with puberty completed. Otherwise, enlargement must be irreversible and have started outside adolescence. Exclusions include obesity, nonprescribed drugs, medicines that can be stopped, incomplete puberty and shorter adolescent duration. Gland under 2 cm, mainly fatty enlargement and liposuction are also excluded.
- What to check
- The gland diameter is this insurer’s requirement, rather than a diagnostic cutoff for everyone. A Michigan Blue policy doesn’t tell you another Blue plan’s benefits.
Policy text is not a coverage guarantee.
BCBS Kansas
Surgical Treatment of Gynecomastia; effective March 15, 2012; reviewed March 24, 2026
- Whose plan
- BCBSKS and self-insured groups using this policy. Other Blue plans and FEP differ.
- Ways to qualify and what’s required
- The policy says the contract excludes reduction, mastectomy and liposuction, and pain doesn’t make them medically necessary. An unusual mass raising cancer concern has an exception for incisional biopsy.
- What to check
- A biopsy checks for disease; cosmetic chest surgery has a different purpose. Check your member benefit certificate.

Blue Shield California BSC7.13
Effective September 1, 2026
- Whose plan
- Blue Shield California plans and the California provisions it cites.
- Ways to qualify and what’s required
- Cancer concerns have separate routes, while the ordinary route requires ALL criteria. You must be over age 18, or 18 months past puberty if younger. You need grade II–IV and gland, with a cause unrelated to puberty, obesity or reversible medicine or substance effects. Records must show that reversible causes were checked and ruled out or treated. Front and side photos must show a significant structural abnormality.
- What to check
- The plan may deny surgery if little appearance improvement is expected or documentation is insufficient. Other grounds include an approved alternative that works as well or better, or a cosmetic purpose. Liposuction done with the reduction is included in its payment, with no separate reimbursement.

Arkansas BCBS 1998074
Reviewed March 2026; the page doesn’t give the new section’s exact effective date
- Whose plan
- Arkansas benefit certificates.
- Ways to qualify and what’s required
- The current main section requires an InterQual medical-necessity decision. As of October 13, 2026, we don’t have its detailed proprietary criteria.
- What to check
- The old age-over-18 section ended March 25, 2026. Ask for the current checklist.
Policy text is not a coverage guarantee.
FEP 7.01.13
Effective July 1, 2026
- Whose plan
- Federal Employee Program Service Benefit Plan. The policy title refers to both breasts.
- Ways to qualify and what’s required
- The policy calls surgery on both breasts not medically necessary because it says function isn’t impaired. It refers to reconstructive benefits but doesn’t give a full gynecomastia exception checklist.
- What to check
- Your member benefits decide; a title about both breasts doesn’t tell you whether there’s an exception for one-sided surgery.
Policy text is not a coverage guarantee.
Kaiser Mid-Atlantic MCP
Implementation August 28, 2024; no numeric policy id; we don’t have a newer version
- Whose plan
- Maryland, Virginia and D.C. Check your own member benefits.
- Ways to qualify and what’s required
- Cancer and androgen-deprivation treatment have separate routes, OR you can meet section V.B. V.B requires enlargement lasting at least 1 year after puberty AND all listed criteria. You need gland unrelated to obesity or puberty, ASPS grade II–IV, and impaired function plus one cause-duration route. Mammography or needle biopsy must show no cancer, specified laboratory tests must be normal, and weight stable for 6 months. The cause-duration routes are treated disease persisting beyond 12–24 months OR unexplained enlargement beyond 24 months after excluding causes. Drug-induced enlargement lasting 6 months after stopping the drug is another route.
- What to check
- BMI over 35 adds at least 3 months of nutrition education, including a professional class and at least 2 individual sessions. This dated regional policy doesn’t tell you nationwide Kaiser rules. Confirm your current member version.
Policy text is not a coverage guarantee.
Kaiser Northwest UR 20.4
Reviewed March 17, 2026; effective 7/09 and revision 2/23 as listed
- Whose plan
- Oregon/Washington commercial and self-funded plans. Medicare uses local policies. Oregon/Washington Medicaid is excluded.
- Ways to qualify and what’s required
- ALL consultation and surgery criteria must be met. These include moderate or marked gland and hormone assessment. Breast and testicular examination must be completed within 12 months. You must have no contributing drugs, steroids or substances within 12 months, or contributing medical disease. Conventional treatment must have failed, and BMI must be at most 34. At age 18, persistence must exceed 12 months; over age 18, it must exceed 6 months. For past tobacco use, you need a quit date over 6 months before referral OR a negative urine anabasine test. The result must be under 3 ng/dl within 30 days if you quit under 6 months ago.
- What to check
- Conventional treatment includes addressing the cause, treating pain or considering 6–12 weeks of tamoxifen. Reasons a surgeon may decline include nicotine or nicotine-replacement use within 30 days, HbA1c at least 8.0 or BMI over 34. Other assessed risks may also rule out surgery. These are this insurer’s criteria. Discuss medicine with your clinician; its weight cutoff doesn’t apply to everyone.

Kaiser Washington KP-0273 v2
Gynecomastia Clinical Review Criteria reviewed June 9, 2026; MCG v2 effective April 1, 2022
- Whose plan
- Kaiser Foundation Health Plan of Washington and Washington Options.
- Ways to qualify and what’s required
- Non-Medicare members use proprietary MCG criteria that aren’t available here. The public policy prefers Plastic Surgery credentials and asks for 6 months of clinical notes. You can request the criteria used for your individual review.
- What to check
- The Medicare table names Noridian L35163 and A57221. Without the proprietary criteria, you can’t tell whether surgery would be approved or excluded.

Authorization and payment
Separate eligibility routes
Cigna’s Klinefelter route is separate from its route requiring a set duration and all listed criteria. Its 12-month hormonal-treatment rule applies only within the latter route. UnitedHealthcare’s June 2026 commercial/Exchange policy says most plans exclude benign gynecomastia treatment, even though it lists adult criteria. Anthem, individual Blue plans, FEP and Kaiser regions have different rules. Check the cards for your plan and policy date.
Authorization and payment
HealthCare.gov says preauthorization decides medical necessity but doesn’t promise payment. It explains appeals and external review without saying how often they overturn exclusions. Get your benefit exclusion, authorization, network status and separate provider charges in writing before paying. Policy text is not a coverage guarantee.

What do Utah insurance policies say about gynecomastia surgery?
For gynecomastia surgery, Regence’s September 1, 2026 policy requires enlargement lasting at least 12 months at age 19 or older. An 18-year-old needs 24-month persistence and Tanner stage IV. These are only part of Regence Surgery 12.06’s rules; other Utah plans differ.
Regence and Select Health in Utah
University of Utah Health Plans and PEHP
More from the sources
Regence also requires gland or fibrous tissue, grade II or higher, cause assessment and treatment, pain or tenderness, and documentation. Select Health’s September 23, 2026 Utah/Idaho Medicare list marks CPT 19300 for preauthorization. Its full clinical checklist isn’t available here.
University of Utah Health Plans’ January 2026 Individual EPO document excludes surgery in section 9.18. That doesn’t tell you what its other plans cover. PEHP’s 2026–27 State guide directs you to your Master Policy. Its older 2024–25 exclusion doesn’t tell you today’s State benefits. As of October 13, 2026, you still need those documents for your own plan. Policy text is not a coverage guarantee.

What does Medicare require for gynecomastia surgery in my state?
Medicare’s gynecomastia surgery rules vary locally; Utah’s Noridian policy names grade III/IV or abnormal development with extra tissue. Utah uses Noridian L35163 and A57221, both revised October 16, 2025. Its gynecomastia paragraph gives no required duration. Other contractors’ billing articles add at least 1 year of persistence, plus treatment and documentation rules.
Noridian, including Utah, and Novitas
WPS and CGS
First Coast
Medicare Advantage documents
More from the sources
Novitas and First Coast articles require a year of persistence with 3–4 months of unsuccessful treatment. WPS and CGS LCDs, or local coverage determinations, list 3–4 months of treatment. Their current billing articles require at least 6 months of unsuccessful treatment and at least 1 year of persistence. Ask how both documents apply to you. Each table names the contractor’s states and both documents.
Humana and UnitedHealthcare Medicare Advantage policies explain which local policy or alternative policy they use. CMS NCD 140.2 concerns reconstruction after medically necessary mastectomy, rather than eligibility for initial gynecomastia surgery. ASPS’s March 2002 recommendation is a society position, rather than an insurer’s rule. Policy text is not a coverage guarantee.
“Which Medicare policy and billing article apply to me, and how will you document every requirement?”
Make the consultation useful
Who performs gynecomastia surgery, and how do I compare different plans?
Before gynecomastia surgery, check board certification; ABPS requires at least 6 years of surgical residency, including at least 3 in plastic surgery. ASPS recommends asking about American Board of Plastic Surgery certification, privileges for this operation and facility licensing or accreditation. “Board-certified” alone doesn’t name the board or tell you about chest-surgery experience.
Check which board certified your surgeon
Compare the proposed operations
More from the sources
39 of 70 patient accounts we read discussed choosing surgeons or comparing consultations. ABPS is an ABMS-member board, while ABCS is a separate certifier and says it has never applied for ABMS membership. A license and ASPS membership are separate checks. A 2018 Arkansas report describes general surgeons’ techniques in 52 adult men, with 70 mastectomies from 2005–2016. It doesn’t compare outcomes by specialty.
Ask a breast or general surgeon which board certified them and how their plan addresses contour, scars and follow-up. When plans differ, first compare findings about gland, fat, skin and nipple position. Then compare the operation, anesthesia, drains, recovery and price.
ASPS says drains may be used. Stanford describes a 6-week vest, while Utah describes a couple of weeks for excision. Taylor & Sullivan restricts chest exercise for 6 weeks. These hospital and practice instructions were checked October 13, 2026. They don’t tell you which operation is better, and the reviews don’t show whether drains improve outcomes.

What should I ask before committing to gynecomastia surgery?
Before gynecomastia surgery, ask for your diagnosis, exact operation, tradeoffs, follow-up and full quote, as the Aesthetic Society recommends. A 2025 US-led review found 23 different patient-reported measures across 22 studies. A satisfaction number can’t tell you how you’ll feel about your own appearance, scars or sensation after surgery.
My gynecomastia surgery consultation sheet
More from the sources
ASPS recommends asking what happens if you’re unhappy with the result and says final results may take 3–6 months. The 2025 review found gaps in quality-of-life and social measures. It doesn’t give a satisfaction rate that represents everyone. SIAMS 2026 suggests checking for distress and offering counseling to some people before and after treatment.
If you regret the decision while others like the appearance, tell your surgeon what troubles you. That could be shape, feeling, recovery or the decision itself. Ask what assessment or support is available. The patient accounts don’t explain why this happens, so they can’t give a diagnosis or promise emotional recovery.
If you travel, ask who assesses new swelling after you fly home and what it costs. For light exercise, ask whether your prescribed vest stays on. Get those details in your written plan.

How we researched this guide
We started with questions people asked about gynecomastia surgery. We checked clinical guidance, grading systems, FDA statements and full insurance policies on October 13, 2026.
Clinical guidance
The answers name ASPS, the Aesthetic Society, Mayo Clinic, Cleveland Clinic, US hospitals, AFP, Endotext, FDA and ACR. They also use EAA 2019 and corrected SIAMS 2026 guidance. Surgical results from other countries don’t tell you your own chances.
What the numbers can tell you
Grading systems, blood-test recommendations and the order of biopsy tests differ. We explain each separately. Counts of complications, revisions and gland regrowth describe different things. Each study’s numbers come with who was counted, when and how long they were followed.
Coverage documents
The tables name each insurer’s plan, document and date. For Original Medicare, read both the local policy and its billing article. We don’t have Select Health’s full clinical criteria or the current PEHP member Master Policy. Policy text is not a coverage guarantee.
Patient questions
On October 13, 2026, we counted 117 of 232 sources, including 70 patient-voice sources. Of those patient sources, 20 were published by practices and 50 elsewhere. One source can describe several people, so these counts don’t tell you how common an experience is. People asked about conflicting plans, surprise bills, follow-up after travel and regret. Where the sources don’t answer, we say so.
The videos
6 stage videos selected from the reviewed list; additional reviewed clips are linked under their chapters, as checked October 13, 2026. Each credits its creator and links to the original. We distinguish patient expectations, clinicians’ explanations and individual results. These clips don’t rank providers or tell you your own chances.
Each video shows one person’s account or a clinical explanation. Its caption explains when it was posted, what was done and whether a result was shown.
