Policy text is not a coverage guarantee.

ASPS describes possible coverage for breathing-related reconstruction after an examination and insurer prior authorization, including when cosmetic surgery is performed in the same session. Your member benefits and the procedure-specific policy decide which work can be submitted. Mayo Clinic says an insurer may pay for some parts of nasal surgery but not others, and its business office can explain the cost. Ask the surgeon to identify the cause of your breathing difficulty and the operation proposed to address it. Then ask the billing office which policy and benefit apply. An office accepting your insurance does not answer those questions. A friend’s covered septoplasty cannot establish coverage for reshaping your bridge or tip. The comparisons below preserve different routes to medical necessity rather than turning them into a single checklist. If you want both a breathing repair and an appearance change, request a written estimate for each before scheduling.

Ask your surgeon

“Which nasal procedures meet my benefit’s medical criteria, and which appearance changes will I pay for?”

Name the operation

Does a deviated septum mean insurance covers a nose job?

A deviated septum does not by itself mean insurance covers a cosmetic nose job. Mayo distinguishes septoplasty, which straightens the internal partition, from rhinoplasty, which reshapes the nose. Aetna and UnitedHealthcare have separate criteria for functional external surgery when septoplasty alone is insufficient.

Internal partition

Internal partition

Septal work

Internal partition
  1. Internal partition

ASPS describes straightening and repositioning the septum. This transverse cutaway shows the partition and two passages, not an airflow measurement.

  2. Septal work

The straighter partition illustrates the aim of septal work, not a promised result. Septoplasty may be done alone or with rhinoplasty. Nasal valve dysfunction can require different treatment; septal work does not substitute for valve correction.

Septoplasty | American Society of Plastic SurgeonsPosition Statement: Nasal Valve Repair - American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS)

Schematics show anatomy and discussion areas, not measurements, a surgical plan, measured swelling reduction or a predicted result. Repeated profiles keep the same contour.

Different structures can need different repairs

Septum

Mayo describes the partition between nasal passages; septoplasty straightens it.

Nasal valve

AAO-HNS says septal and turbinate procedures can complement valve repair but cannot substitute for correction of valve dysfunction.

External shape

Aetna and UHC ask why septoplasty alone cannot correct the obstruction under their functional rhinoplasty branches.

The drawing shows where septal work happens. It cannot diagnose obstruction or measure the airflow you would gain. AAO-HNS distinguishes nasal valve dysfunction from septal and turbinate problems. Correcting a septum can complement valve repair, but the society says it cannot substitute for correcting valve dysfunction. Ask which structure is involved and why each proposed procedure is needed. That explanation helps the office match the request to the appropriate policy branch. It also prevents a combined septorhinoplasty quote from being mistaken for the cost of isolated septoplasty. A Cottle maneuver, photograph or scan has a specific purpose within an examination; none is a universal coverage test. Some policies ask for imaging of external or bony obstruction, while clinical guidance on diagnosing septal deviation does not require the same evidence. Ask which documentation is for diagnosis and which is required by the payer. Keep optional bridge or tip changes identified separately.

Ask your surgeon

“Is my obstruction septal, valve-related, external or combined, and why is each proposed repair needed?”

When does Aetna cover rhinoplasty or septoplasty?

Aetna CPB 0005, last reviewed March 12, 2026, lists alternative septoplasty indications and a separate functional rhinoplasty pathway. Its septal-obstruction branch requires at least four weeks of appropriate therapy. That duration is not a requirement for every septoplasty indication.

Match the operation, policy and benefit

Aetna · CPB 0005Last review 03/12/2026; effective 07/31/1995. Checked October 16, 2026.

Procedure and jurisdictionSeptoplasty. Commercial; contract controls.

Criteria in the checked policyANY: asymptomatic deformity prevents necessary intranasal access; recurrent sinusitis due to septum despite appropriate medical/antibiotic therapy; septal epistaxis; continuous obstruction/breathing difficulty despite >=4 weeks appropriate therapy. Cleft repair is a separate out-of-family indication.

Documentation and exclusionsDocument septal cause and conservative treatment; benefit terms apply. Other indications unproven; cosmetic rhinoplasty otherwise excluded.

Aetna · CPB 0005Last review 03/12/2026. Checked October 16, 2026.

Procedure and jurisdictionFunctional rhinoplasty / combined septorhinoplasty. Commercial; contract controls.

Criteria in the checked policyIndividual review vestibular stenosis from trauma/disease/congenital defect: prolonged persistent obstruction; exam moderate/severe vestibular obstruction; septoplasty/turbinectomy alone insufficient; significant symptoms; >=4 weeks conservative treatment; external deformity photos; endoscopy/CT/other imaging significant obstruction. Alternative: integral to necessary septoplasty with gross obstruction on same side as deviation.

Documentation and exclusionsDuration/degree, treatment results, 4-way photos, relevant history, endoscopy/CT/other appropriate imaging per footnote. All applicable criteria, not just one symptom.. All other rhinoplasty cosmetic; extracorporeal revision septoplasty unproven.

Policy text is not a coverage guarantee.

Aetna’s septoplasty alternatives include specified sinusitis, septal bleeding and access problems. The obstruction route requires continuous breathing difficulty attributable to the septum despite the stated treatment. For vestibular-stenosis rhinoplasty, the listed requirements work together: persistent symptoms, examination findings, insufficient correction by septoplasty or turbinectomy alone, treatment where appropriate, photographs and documentation of significant obstruction. Picking a symptom does not satisfy the whole branch. The policy separately describes rhinoplasty integral to medically necessary septoplasty with gross obstruction on the same side as the deviation. Ask the office to identify its route rather than blending that alternative with the vestibular requirements. Aetna treats other rhinoplasty indications as cosmetic and benefit terms still apply. Its bulletin also classifies some valve devices or suspension procedures as unproven; a society’s different view of a technology is not an authorization from Aetna. Confirm the exact operation and version before relying on an estimate.

When does Cigna cover rhinoplasty or septoplasty?

Cigna 0119, effective July 1, 2026, has no universal conservative-treatment duration in its current septoplasty criteria. It lists breathing difficulty or mouth breathing due to septal deviation or deformity, recurrent septal bleeding and a specified CPAP-related pathway. Functional rhinoplasty and vestibular repair have separate requirements.

Match the operation, policy and benefit

Cigna · 0119Effective 7/1/2026; next review 5/15/2027. Checked October 16, 2026.

Procedure and jurisdictionSeptoplasty. Cigna administered plans; contract controls.

Criteria in the checked policyANY: breathing difficulty/mouth breathing due to septal deviation/deformity; recurrent septal epistaxis; obstructed breathing interferes with necessary CPAP for OSA with AHI >=15 on polysomnography/home study. Covered cleft repair is separate source indication. No universal conservative-therapy duration in current septoplasty criteria.

Documentation and exclusionsDocument cause; sleep study for CPAP pathway; plan determines authorization. Nonlisted indications not necessary; balloon dilation septoplasty unproven.

Cigna · 0119Effective 7/1/2026. Checked October 16, 2026.

Procedure and jurisdictionFunctional rhinoplasty / vestibular stenosis. Cigna administered plans.

Criteria in the checked policyTrauma-related deformity with functional impairment: frontal/lateral/base photos; unresolved after septoplasty/turbinectomy or not expected to resolve with those alone; rhinoplasty expected to resolve impairment. Vestibular stenosis: chronic obstruction and improved airway on Cottle OR internal upper-lateral-cartilage lateralization. Congenital pathway separate in full source.

Documentation and exclusionsPhotos and exam; contract controls; source not a billing allocation formula. Appearance-only and primary OSA treatment without criteria cosmetic/not necessary; absorbable implants/RF unproven.

Policy text is not a coverage guarantee.

The recheck found that Cigna removed its recent conservative-treatment requirement in October 2025. Do not import Aetna’s duration into this version. The CPAP branch requires septal obstruction interfering with medically necessary CPAP and documented sleep apnea with AHI at least 15 on the named type of sleep study. That is a particular indication, rather than coverage for everyone who uses CPAP. For traumatic functional deformity, Cigna asks for photographs, impairment that septoplasty or turbinectomy has not resolved or is not expected to resolve alone, and expected correction through rhinoplasty. Its vestibular-stenosis route requires chronic obstruction with improvement through Cottle maneuver or internal lateralization of upper lateral cartilage. The card keeps those routes separate. Cigna says coverage varies by plan, including cosmetic and reconstructive benefits and mandates. Appearance-only surgery remains excluded. Treating a sleep disorder without the listed criteria does not establish medical necessity. Ask which procedure and indication the office will submit under your contract.

Ask your surgeon

“Which current Cigna indication supports my request, and are any sleep-study or photograph requirements missing?”

When does UnitedHealthcare cover functional or revision rhinoplasty?

UnitedHealthcare MP.019.34, effective April 1, 2026, requires all applicable criteria for its functional primary, revision, tip or valve-repair branch. Its primary, revision and tip passages say at least four weeks of conservative management; its definition of prolonged, persistent obstruction separately specifies six weeks. Both passages must be read together.

See complete alternative indications in policy.

Documentation and exclusionsAll applicable criteria and member benefit terms control. Criteria explicitly state at least four weeks in primary/revision/tip pathways; the defined term Prolonged, Persistent Nasal Airway Obstruction separately requires six weeks of medical management and elimination of drug-induced rhinitis. Preserve both passages; do not simplify to universal four weeks.. Other primary indications not reconstructive/necessary.

Tip: prolonged persistent obstruction due to tip drop, photos/acute columellar-labial angle, lift plan, significant symptoms and >=4 weeks conservative management.

Revision/tip primarily cosmetic unless ALL respective criteria satisfied.

Criteria in the checked policyALL: prolonged persistent obstructed breathing; other causes treated maximally; septal/turbinate issues absent, previously treated or planned concurrently; visible collapse on deep inspiration; subjective/audible modified Cottle improvement; photos match exam; surgeon describes static/dynamic, internal/external and clear graft/surgery plan.

All other valve repair indications not necessary.

Policy text is not a coverage guarantee.

This is the Commercial and Individual Exchange policy, not a substitute for another UnitedHealthcare product. The primary branch asks why septoplasty alone cannot correct obstruction, with matching photographs, significant symptoms and a corrective plan. The revision branch concerns persistent functional impairment from a complication or residual deformity of prior functional surgery; dissatisfaction with appearance does not establish that branch. Tip-related coverage concerns mechanical tip drop with its own findings and correction. Valve repair has separate requirements, including visible collapse during inspiration, subjective and audible improvement with modified Cottle maneuver, matching photographs and a clear description of the repair and graft needs. Relevant septal or turbinate problems can be absent, previously treated or planned for concurrent treatment as specified. Ask how other causes of obstruction have been addressed. The policy’s defined terms matter as much as its bullet points. A positive maneuver alone is insufficient, and no single treatment duration replaces the full applicable language.

Does Anthem cover combined septoplasty and rhinoplasty?

Anthem’s combined septorhinoplasty request must satisfy both CG-SURG-18 for septoplasty and ANC.00008 for rhinoplasty. The checked versions were published April 15, 2026, and October 1, 2026, respectively. Meeting the septoplasty criteria alone does not establish coverage for the external operation.

Criteria in the checked policySymptomatic septal deviation/deformity AND >=1 distressing obstruction without another likely treatable cause, recurrent/persistent epistaxis, chronic/recurrent acute sinusitis AND failed reasonable conservative management. Separate access pathway: deformity prevents necessary intranasal/paranasal access.

Documentation and exclusionsDocument failed treatment; combined rhinoplasty also must meet ANC.00008. Other indications not medically necessary.

Anthem / Elevance · ANC.00008Review 08/13/2026; publish 10/01/2026. Checked October 16, 2026.

Procedure and jurisdictionRhinoplasty / septorhinoplasty. Applicable Anthem plans.

Criteria in the checked policyBOTH: documented failed conservative therapy for severe obstruction due to disease/structural abnormality/previous therapeutic process not responding to septoplasty alone; expected functional improvement. Combined septorhinoplasty also meets CG-SURG-18. Initial restorative repair only unless staged.

Documentation and exclusionsMedical record; reconstructive fracture pathway in source outside standalone family topic. Shape/size modification without criteria cosmetic/not necessary.

Policy text is not a coverage guarantee.

CG-SURG-18 describes symptomatic septal deviation or deformity with a qualifying problem and failed reasonable conservative management. It also has a separate access indication. Its operative criteria do not supply a fixed duration to copy from another insurer. ANC.00008 requires documented failed conservative therapy for severe obstruction that will not respond to septoplasty alone and a reasonable expectation of functional improvement. Its initial restorative-repair and staged-repair language matters for someone who has already had surgery. Ask the office to read that provision before assuming a repeat operation qualifies. These documents apply to adopting plans and their benefit terms; they are not a nationwide member contract. A separate Anthem device policy treats specified suspension, absorbable implants and radiofrequency procedures as investigational. That does not exclude every conventional valve reconstruction. If a proposed treatment has a device name, ask which document governs it as well as which document governs the septum or external nose. Keep both decisions with the combined estimate.

Ask your surgeon

“Does my combined request satisfy both Anthem policies, including their restorative-repair and benefit provisions?”

The Regence document served in the research is effective August 1, 2025. Its symptomatic-obstruction route requires the listed bony obstruction, imaging, inadequate correction by alternative procedures and treatment or documented circumstances where treatment is not indicated. Noridian’s L35163 revision is effective October 16, 2025. The companion audit did not verify a current contractor-state list, so confirm your contractor and location before applying it. Noridian describes potentially reconstructive respiratory, congenital, disease and traumatic repairs; appearance-only surgery without functional abnormalities is noncovered. Benefits cover only the covered procedure in a mixed session. Blue Cross NC expressly excludes septoplasty performed as part of a cosmetic reconstructive procedure; a broad educational promise of partial coverage cannot replace that wording. BCBS Massachusetts 068 addresses specified obstruction unresponsive to septoplasty alone or reconstruction, with initial-repair restrictions and authorization provisions. Regional criteria cannot be generalized to every Blue Cross plan. Ask for the actual document your plan uses.

PEHP’s questionnaire asks about symptoms, treatment, photographs, tests and whether septoplasty or turbinectomy could resolve obstruction. Its questions do not establish a complete current eligibility algorithm. Ask for the policy and benefit language used for your request rather than treating the form’s treatment period as a current rule. The Select Health and University of Utah Health Plans portals did not return procedure-specific criteria in the companion audit. The Humana portal did not establish commercial criteria either. That is an access gap. A Medicare Advantage document is a separate product and should not be relabeled as Humana commercial coverage. Similarly, an archived device booklet cannot establish a current plan’s coverage of all functional nasal surgery. Ask the plan and office to identify the applicable criteria, version date and authorization requirement. Keep a reference with the quote so later questions can be tied to the document actually used. Do not fill the gap with another payer’s duration or examination threshold.

Ask your surgeon

“Can my plan provide the current nasal-surgery policy and benefit language used to decide this request?”

How is a cosmetic and functional rhinoplasty bill split?

No fixed cosmetic-versus-functional percentage split is established by these rhinoplasty policies. Mayo says an insurer may pay for some parts of nasal surgery and not others. Ask for the medical submission, cosmetic self-pay quote and expected patient cost separately in writing.

No fixed percentage split is established by the research. Policy text is not a coverage guarantee.

Noridian’s mixed-session rule provides benefits only for the covered procedure. Blue Cross NC’s exclusion also shows why a broad description of partial coverage cannot settle every combined case. Ask how the office allocates surgeon, anesthesia and facility charges, and which amounts remain your responsibility if the insurer pays less than expected or denies the claim. Include deductible, copay or coinsurance questions when contacting your plan. The selected surgeon explanation distinguishes those patient costs from cosmetic self-pay; it does not supply your benefit calculation. A deposit is different from the final insurer payment. Before paying it, request written terms for authorization, denial, cancellation and refunds. Do not subtract a hoped-for insurance amount from the cosmetic quote and treat the remainder as settled. The worksheet records questions the sources and patient discussions leave unresolved for an individual bill. Use the main cost guide to compare the underlying fee inclusions.

Ask your surgeon

“Can you show my medical, cosmetic and patient-share estimates, including what I owe if payment is denied?”

Planning prompts, not an appeal strategy. No approval probability, appeal success rate or standard decision deadline was established.

Mayo warns that prior written authorization is not a coverage guarantee. Keep it with the estimate and ask what financial responsibility remains if the claim is denied. Missing documentation raises a different question from a benefit excluding the operation. Have the office explain which issue the letter identifies before discussing the next step. For combined surgery, check which component the decision names; an answer about septoplasty does not necessarily decide rhinoplasty, valve repair or cosmetic work. Do not add symptoms to fit a policy checklist. Describe what you actually experience and let the clinician document the examination and treatment history. The worksheet organizes an accurate discussion; it is not a tested appeal strategy. If the proposed operation changes, ask whether the office needs a new request and estimate. For care and practical recovery questions while planning, use the family’s recovery guide and your team’s own instructions.

Ask your surgeon

“What exactly was denied, what does the plan ask for next, and what would I owe under each outcome?”

How we researched this answer

We used the corrected clinical claims and KEEP/FIX companion insurance records in the supplied packet, checked October 16, 2026. Patient-source questions about approval, separate billing, surprises and missing criteria shaped this outline. Their partial discussions and provider explanations do not establish approval rates or reconciled individual bills. Every policy card preserves its identifier, date, jurisdiction, procedure and alternative or combined criteria and links to the full document. Future dates, older forms and portal gaps remain visible. No fixed allocation or appeal outcome was established. Worksheets are editorial prompts, not payer requirements. Selected KEEP videos render only through playableIds after playback is available and illustrate distinctions rather than replacing policy text.
