The answer
How do I get insurance to cover a panniculectomy?
To request insurance coverage for panniculectomy, identify your exact plan’s benefit and policy, have the surgical team document the medical problem and required treatment history, and ask the office to submit the evidence the plan requires. ASPS says panniculectomy may sometimes be covered and advises consulting the insurer before surgery. Policy text is not a coverage guarantee.

The pannus is excess hanging skin and fat, shown from the front.

Schematic anatomy and care topics, not measurements, predicted scars, results or a personal recovery schedule.
There is no approval phrase or universal rash checklist. The request needs to describe your actual condition and the operation being proposed. ASPS defines panniculectomy as removing the hanging apron of skin and fat without fascial plication, creation of a new umbilicus or flap elevation. Additional contouring changes that scope and can remain cosmetic even when apron removal is covered. Start with your member benefit rather than a different patient’s approval story. Ask the plan which medical policy it uses, whether a benefit exclusion applies and whether authorization is required for the proposed setting. Have the office explain how its request addresses that document. The policy comparisons below preserve product limits and important conjunctions; meeting a fragment from a table is not the same as satisfying the complete contract. Do not schedule on a verbal expectation of coverage without understanding the written decision and your financial responsibility.
“Which exact benefit and medical policy apply to my panniculectomy request, and what documentation is still needed?”

What records support a panniculectomy insurance request?
A panniculectomy request should document the symptoms, clinical findings and treatment response your own policy requires. Aetna asks for specified photographs and chronic intertrigo treatment history. Cigna also requires a functional deficit, expected improvement and interference with daily activities in the history and physical.
Records to review with the office
Describe the activities that are difficult, when skin problems occur and which clinician has assessed them. Ask the office to obtain dated treatment records and record the response, including recurrence where relevant. A photograph showing the apron does not by itself document treatment failure, and a list of medicines does not explain what happened during treatment. Aetna requests frontal and side color photographs and lifted-pannus views documenting intertrigo. Other payers use different language, so ask which views and records apply to your request. Do not add symptoms because they appear in a qualifying list or use another person’s medication history as your own. Hygiene concerns can be relevant to your consultation without satisfying every payer’s medical-necessity pathway. Needed care should follow the treating clinician’s assessment; this worksheet is for organizing an accurate record, not for waiting out a rash or creating evidence. Ask the team to identify the missing item before paying for more documentation.

What do commercial plans require for panniculectomy coverage?
Aetna, Cigna, UnitedHealthcare, Anthem and Blue Cross NC use different panniculectomy criteria. The cards name the document and version and retain the distinctions between required combinations and alternative pathways. A rule from another insurer or another Blue plan cannot establish your eligibility.
Commercial plans use different criteria
The most consequential wording is often AND or OR. Aetna’s conditions work together. Cigna requires functional documentation alongside its skin and anatomic criteria. Anthem allows a skin-disease route or a functional route, but the functional route requires walking difficulty and interference with daily activities together. Its weight-loss and bariatric clauses remain separate. UnitedHealthcare refers to InterQual and does not publish the numerical criteria in the public document read; missing criteria cannot be filled with Aetna’s thresholds. Blue Cross NC’s ALL introduction and OR bullets should be interpreted by that payer in their original layout. These cards are summaries to help you find the applicable source and ask an informed question. They cannot replace the full document, a benefit exclusion or the plan’s assessment of the submitted record. Confirm which policy your plan adopts and which version applies to the proposed date of service.
“Which parts of my plan’s criteria must all be met, and which are genuine alternative pathways?”
Your payer
What do Utah plans require for panniculectomy insurance?
The verified Utah documents support a current Regence policy comparison, but they do not establish complete current eligibility for every SelectHealth, PEHP or University of Utah Health Plans member. SelectHealth’s complete current policy was unconfirmed, PEHP’s public form is incomplete, and the UUHP document covers a specific individual EPO product.
Utah documents and the limits of what was verified
Regence requires the listed conditions together, including skin disease, functional interference and weight stability. Its main criterion and documentation checklist differ on systemic antifungal wording; ask Regence to resolve that discrepancy rather than deciding it yourself. SelectHealth’s archived policy and later bulletin answer different questions. The archive preserves historical criteria, while the bulletin announces noncoverage language without restating complete eligibility or specifying the change’s effective date. PEHP’s questionnaire asks clinically relevant questions but does not disclose the full separate panniculectomy policy. A checked box is not proof of coverage. The UUHP individual EPO lists specific exclusions alongside broader reconstructive exceptions; ask the plan to interpret that relationship for your benefit. If you have a different UUHP product, this document cannot determine its terms. The cards keep each limit attached to the source so that a search result cannot become an unsupported Utah-wide rule.

Which Medicare policy applies to panniculectomy in my state?
The Medicare panniculectomy criteria in this research belong to local contractors rather than a national approval checklist. Noridian’s jurisdiction includes Utah. Novitas, Palmetto, WPS and CGS publish different local requirements, and these reviewed policies do not cover every state or every Medicare Advantage product.
Find the applicable local contractor
Find the contractor and jurisdiction before copying a weight-stability period from a search result. Noridian’s reviewed text describes refractory functional symptoms and possible photograph requests without importing another payer’s BMI or stable-weight threshold. WPS and CGS retain recent infection or inflammation in their weight-loss timing paragraph, so stable weight alone is incomplete. The cards preserve those differences and their effective dates. Medicare Advantage introduces a separate product question. Humana’s Medicare Advantage policy HUM-1064-006, effective October 1, 2026, applies local MAC guidance to Part A and Part B requests in the listed jurisdiction; its Utah entry points to Noridian. Its fallback for panniculectomy in J6/JK refers to WPS. That fallback is Humana’s rule, not proof that Original Medicare in those jurisdictions automatically adopts WPS. Ask the plan and office which document controls your request and surgical setting.
“Which local contractor or Medicare Advantage policy governs my state, product and place of surgery?”
Weight-loss timing
How long must my weight be stable for panniculectomy insurance?
Panniculectomy weight-stability requirements vary by payer, and their clocks start at different events. Aetna’s bariatric interval is tied to plateau, while Cigna and several local Medicare policies specify postoperative timing. Anthem’s reviewed clause uses an alternative. None should be rewritten as a universal waiting period.
Keep the starting point attached to the waiting period
The bariatric interval is at least twelve months since plateau, not twelve months after surgery. Latest three-month weight stability is a separate condition.
Its postoperative clause says eighteen months OR three stable months. Its separate weight-loss and symptom criteria still apply.
Their weight-loss timing paragraph also retains infection/inflammation during the latest three months. A calendar alone does not establish eligibility.
Keep a dated weight history and any bariatric surgery history available for the office. Then ask the plan which interval applies, how stability is defined and what else must be present. Aetna’s stable-weight condition and its time since bariatric plateau are separate requirements. Anthem’s postoperative alternative does not erase its separate symptom and weight-loss criteria. WPS and CGS also require infection or inflammation during the latest three months in the relevant timing paragraph. The comparison cards preserve the actual periods with their source; the shorter card here highlights why the starting event matters. Medical readiness for an elective operation is another question. Meeting a benefit’s calendar requirement does not establish that nutrition, smoking risk or ongoing weight change have been addressed. ASPS’s massive-weight-loss parameter includes nutritional assessment and smoking cessation planning, with delay or limitation of surgery considered when risks cannot be addressed.
“What event starts my plan’s stability clock, what records prove it, and what clinical readiness questions remain?”
Covered and cosmetic work
Can panniculectomy insurance also cover a tummy tuck or muscle repair?
Coverage for panniculectomy does not automatically cover cosmetic tummy tuck work or fascial plication. Noridian says the cosmetic portion stays noncovered during covered surgery. The dated ASPS billing recommendation and Humana Medicare Advantage’s conditional abdominoplasty language differ and need interpretation within their own policy settings.
Before plication
Separated rectus muscles with connective tissue between them; simplified transverse cross-section.
Fascial sutures
Plication folds and sutures the fascia. The drawing does not show muscle being cut or removed.
Schematic anatomy and care topics, not measurements, predicted scars, results or a personal recovery schedule.

The cross-section explains why repair is an additional step rather than another name for apron removal. ASPS’s recommendation, reapproved in March 2019 with coding updated in January 2019, distinguishes panniculectomy code 15830 from the cosmetic abdominoplasty add-on +15847 and recommends not billing the cosmetic code to insurance. That society recommendation is not a payer contract or current authority for its older hernia-code examples. Humana’s Medicare Advantage document says abdominoplasty may qualify with medically necessary panniculectomy as defined by jurisdictional guidance, while retaining cosmetic exclusions. Ask the payer and billing team to explain that conditional wording for the actual request. Do not infer that the code will be paid. UnitedHealthcare’s commercial discussion allows a cosmetic addition without granting it coverage. Ask for each proposed operation and the cosmetic balance in writing; a combined session should not obscure the distinction.
“Which procedures are you requesting as covered, which remain cosmetic, and what written policy supports the billing plan?”
The request
What should I confirm before submitting panniculectomy authorization?
Before a panniculectomy request is submitted, ask the office to match the procedure, benefit and supporting records to the policy your plan actually applies. Confirm any authorization requirement for the proposed setting with the plan. A procedure code appearing in a policy does not establish payment.
Before the office submits the request
Use the worksheet to keep the policy name and version with the request. Ask who sends photographs and treatment records, who answers a missing-information request and how you receive the written decision. The office should be able to distinguish the documented clinical indication from optional contouring work. Have the plan explain benefit exclusions and patient-share questions rather than assuming the medical policy settles both. Network participation, facility billing and anesthesia billing also deserve direct answers before scheduling; this packet does not calculate an individual insured balance. If the team describes an authorization as approval, ask what that decision establishes and which conditions or exclusions remain. Keep copies of the response and the financial agreement. This is an editorial planning process based on the policy distinctions in the research, with no standard processing duration or promise that a complete packet will be approved.
“Who will submit and track the request, and can I receive the policy reference, written decision and patient-responsibility explanation?”
After the decision
What can I do if panniculectomy insurance is denied?
If panniculectomy coverage is denied, obtain the written reason and ask the office to identify the benefit provision, policy and evidence used in the decision. Follow your plan’s reconsideration or appeal instructions and deadline. The verified research does not establish an appeal success rate or a standard time to approval.
If the plan denies the request
A missing treatment record, a different interpretation of functional impairment and an excluded benefit require different questions. Ask which issue the letter describes before paying for another consultation or test. Have the office check whether the request named the intended operation and supplied the evidence the applicable document requires. If the dispute concerns contract language, ask the plan to identify the exact provision rather than relying on an office’s general expectation. This worksheet is a way to organize your next conversation, not a legal argument or an appeal template with a promised result. Do not add symptoms or reconstruct a treatment history that did not occur. Also ask what happens to deposits and scheduling while the issue is unresolved and what you would owe if you elect self-pay surgery. Keep the decision, next-step instructions and any later response together so the people helping you can work from the same record.
“What exactly was denied, what can the office accurately clarify, and what next step and deadline does my decision letter give?”
After the decision
What will I pay even if panniculectomy insurance is approved?
A panniculectomy authorization does not establish your final out-of-pocket amount or pay for every cosmetic addition. Ask the plan for your benefit responsibility and the office for the separate patient-pay work. ASPS lists surgeon, facility, anesthesia, medication, garment and testing charges as possible panniculectomy cost components.
Ask what you would actually owe
Ask who bills each component and whether the financial agreement addresses a denied or unpaid claim. For a combined session, request an explanation of how the covered and cosmetic portions are separated, including shared services. Noridian’s rule keeps cosmetic surgery noncovered even in the same session; the covered operation should not be used as a blanket explanation for the rest of the bill. A practice’s self-pay price is also different from a billed insurance charge, an allowed amount or your remaining balance. The full panniculectomy cost guide preserves the self-pay scope and fee bases rather than calculating an insured person’s bill from advertisements. Ask about follow-up, complication care and later revision responsibility before agreeing to the plan. You can use the worksheet even when amounts remain unknown, as long as the office and plan identify which questions each can answer.
“Can I have a written explanation of my plan share, separate cosmetic charges and responsibility if any claim is unpaid?”

How we researched this answer
We used corrected claims and policy context checked October 16, 2026, preserving payer identifiers, effective or review dates, jurisdictions, products and AND/OR logic. The five local Medicare documents are not a national inventory. Current full SelectHealth criteria, the complete PEHP clinical policy and UnitedHealthcare InterQual details remain unverified; UUHP evidence is one individual EPO. Commercial Humana criteria were not established. The supplied voices questions about records, denial and surprise bills shaped this workflow; their incomplete pass and patient stories cannot establish eligibility or appeal outcomes. Worksheets are editorial planning tools. No video supplies coverage evidence or is assigned to this page.
