Candidacy and insurance: Breast Reduction: Will Insurance Cover It?

Breast reduction removes tissue and reshapes the breasts. ASPS links it to neck, back and shoulder pain, skin irritation, strap grooves and activity limits. Insurance may cover symptomatic reduction when your benefit and policy criteria are met. Compare the operation, expected benefits and permanent trade-offs before deciding.

Understanding breast reduction

What is breast reduction surgery?

Breast reduction, also called reduction mammaplasty, removes breast skin and tissue, then reshapes and elevates the breasts, according to ASPS guidance checked October 17, 2026.

What is breast reduction surgery

Remove

Breast tissue and excess skin are removed. The amount and distribution depend on the surgical plan.

Reshape and elevate

The remaining breast is reshaped and the nipple position addressed. Reduction includes a reshaping step.

Set limits

A smaller breast, a particular bra label and complete pain relief are different goals. None follows automatically from a chosen number of grams.

The decision starts with what you want to change. ASPS lists neck or back pain, shoulder grooves, skin irritation and limitations on physical activity among reasons to consider reduction. Wanting smaller breasts for comfort, appearance or both also belongs in the conversation. Those goals do not by themselves establish insurance eligibility. A surgeon needs to examine your anatomy and discuss what tissue can be removed while preserving a viable breast and nipple.

Ask your surgeon

“What can breast reduction change for my symptoms, size and shape, and what would remain?”

Understanding breast reduction

Does breast reduction help back and neck pain?

Breast reduction improved pain and health status in Collins and colleagues’ 2002 prospective BRAVO-related study of 179 operative patients with matched before-and-after data, compared with 96 normal controls and 88 hypertrophy controls.

Does breast reduction help back and neck pain

BRAVO prospective comparison, 2002

179 operative patients with matched data; 96 normal and 88 hypertrophy controls. Follow-up questionnaires at 6–9 months.

BREAST-Q cohort, 2021

238 complete paired responses. Symptom and satisfaction scores are different from an insurer’s gram requirement.

Randomized-trial review, 2021

7 trials, 285 participants; 4 studies pooled. Score improvements do not predict an individual response.

BRAVO means Breast Reduction Assessment of Value and Outcomes. Participants answered follow-up questionnaires about 6–9 months after surgery. Benefit was not associated with body weight, bra size or resection weight in that study. It was not a randomized trial, and the predominantly white, middle-aged, educated and employed sample limits transfer to every patient. Persistent symptoms can have other causes; this evidence cannot promise your pain will resolve.

Ask your surgeon

“Which of my symptoms are likely related to breast weight, and how will we measure improvement?”

Understanding breast reduction

Who is a candidate for breast reduction?

Breast reduction candidacy depends on symptoms, health and healing risks, and ASPS’s 2022 guideline recommends risk counseling rather than automatic exclusion for age over 50, BMI above 35 or chronic corticosteroid use.

Who is a candidate for breast reduction

Clinical assessment

Symptoms, other causes of pain, breast anatomy, previous procedures and your goals.

Healing assessment

Nicotine exposure, medical conditions, medication review and individualized preparation.

Coverage assessment

Your actual benefit and policy version. A BMI requirement in one document does not apply to all plans.

This guide addresses adults. ASPS’s candidate page lists nonsmokers and non-vapers, and its guideline recommends referral for nicotine cessation without prescribing one universal cessation interval. Tell the surgeon about cigarettes, vaping, nicotine products, medicines, supplements, alcohol, previous surgery and conditions affecting healing. Changes to prescribed treatment need coordination with your surgical and prescribing teams.

Ask your surgeon

“Which health factors change my breast reduction risks, and what needs to happen before we schedule?”

Understanding breast reduction

What scars does breast reduction leave?

Breast reduction leaves permanent scars, commonly an inverted-T or Wise pattern or a vertical pattern, according to ASPS and Cleveland Clinic guidance checked October 17, 2026.

What scars does breast reduction leave

Wise or inverted-T

Around the areola, vertically downward and horizontally in the fold. The fold component allows a different skin-removal pattern.

Vertical

Around the areola and vertically downward, without the horizontal fold component.

Permanent lines

Scars usually change over time. Their final width, color and texture cannot be read from an early photograph.

Cleveland Clinic describes the anchor pattern as an incision around the areola, down to the fold and horizontally along the fold. The vertical or short-scar pattern circles the areola and runs down without the horizontal fold incision. ASPS says anatomy, tissue composition, desired reduction, preferences and surgical judgment determine technique. A shorter incision is not automatically the right choice for the skin that needs to be removed.

Ask your surgeon

“Which breast reduction scar pattern fits my skin and reduction goals, and why?”

Understanding breast reduction

Will breast reduction detach my nipples?

Breast reduction usually repositions the nipple on attached tissue carrying blood and nerve supply, while ASPS describes a free nipple graft as a different option for some very large breasts in guidance checked October 17, 2026.

Will breast reduction detach my nipples

Attached pedicle

A nipple-bearing bridge of tissue remains connected. Discuss blood supply, ducts and nerves separately.

Free nipple graft

The nipple-areola is detached and replaced as a graft. Consent needs to address lactation and sensation consequences.

Incision is a separate choice

The outside scar does not prove how the nipple was supported internally.

The attached tissue is called a pedicle. A free nipple graft detaches and replaces the nipple-areola as a graft, with sensory loss and longer healing among ASPS’s stated trade-offs. The choice needs an individual explanation. A circular scar around the areola cannot tell you whether the nipple was detached. Ask about both the planned technique and circumstances that could change that plan during surgery.

Ask your surgeon

“Would you keep my nipple attached to a pedicle, and under what circumstances might a free nipple graft be needed?”

Understanding breast reduction

Does breast reduction include a lift or liposuction?

Breast reduction includes reshaping and elevation in ASPS’s description, while Cleveland Clinic limits liposuction-only reduction to selected breasts with mainly excess fat and no extra-skin problem in guidance checked October 17, 2026.

Does breast reduction include a lift or liposuction

Reduction with reshaping

Volume removal plus skin tailoring and elevation.

Additional liposuction

Name the area, expected effect, separate cost and recovery implications.

Liposuction only

Selected predominantly fatty volume with no excess-skin problem; evidence and coverage differ.

A lift and a reduction overlap in reshaping but have different volume goals. Side or axillary liposuction may be discussed as additional contour work; ask exactly which area it treats and whether it is part of the surgical quote. Liposuction-only reduction removes fat without the same skin-excision pattern. It does not automatically achieve the same size, shape or skin change as excisional reduction.

Ask your surgeon

“What reshaping is included in my reduction, and would liposuction treat a separate area or goal?”

Insurance and the decision

Does insurance cover breast reduction?

Insurance can cover breast reduction for qualifying symptoms when the benefit exists, but Cigna policy 0152, effective November 15, 2025, says the member’s benefit document overrides conflicting policy information.

Commercial plans use different pathways

Aetna · CPB 0017Effective October 6, 1995; reviewed March 12, 2026; next review January 14, 2027.

Symptoms and treatmentAll macromastia conditions must be met. Symptoms in at least 2 listed areas must affect daily activity for at least 1 year. Areas include headache, neck, shoulder or upper-back pain, x-ray-confirmed painful kyphosis, painful or ulcerating strap grooves, skin breakdown and upper-extremity paresthesia. Symptoms must be primarily attributable to macromastia, with likely improvement after surgery and physician-documented pain despite at least 3 months of therapeutic measures.

Tissue and ageAge 18 or completed growth, defined as stable breast size over 1 year. Each breast must meet Aetna’s own Mosteller-BSA appendix for breast tissue, not fatty tissue. More than 1 kg per breast is an alternative with the symptom criteria still met.

Documentation and workflowHigh-quality color frontal and side photographs documenting severe hypertrophy. Negative mammogram within 2 years if age 50 or older. Measures listed include medicines, skin care, therapy/exercises/posture, supportive devices, chiropractic/osteopathic care, supervised weight loss and spine evaluation; not mandatory physical therapy alone.

Limits to applying this policyIntertrigo alone is insufficient; at least 6 months of refractory dermatologic and conservative treatment plus the other criteria are needed. Asymptomatic reduction is cosmetic; liposuction-only is unproven. The accessed pathway prints no general numeric BMI ceiling or complete authorization workflow.

Cigna · 0152Effective November 15, 2025; printed next review August 15, 2026 has passed. Confirm current adoption.

Symptoms and treatmentAll conditions apply if the benefit exists. At least 1 qualifying condition unresponsive to medical management: shoulder, upper-back/neck pain or ulnar palsy after other causes are excluded, or inframammary intertrigo, dermatitis, eczema or hidradenitis. The accessed section gives no fixed treatment duration.

Tissue and ageAge at least 18 or completed breast growth. Average grams per breast above the 22nd Schnur percentile, or more than 1 kg per breast regardless of BSA.

Documentation and workflowPreoperative photographs confirm significant hypertrophy and any reported grooving or intertrigo. The benefit document supersedes policy language. No universal BMI limit or complete authorization route is printed in this section.

Limits to applying this policyAppearance-only or psychological complaints without qualifying physical findings are cosmetic. Sole-method liposuction is unproven. Nipple reconstruction and liposuction are integral to the reduction code, not separately reimbursable under this policy.

UnitedHealthcare · MP.004.32Commercial and Individual Exchange; effective June 1, 2026.

Symptoms and treatmentMost plans exclude reduction except where legally required; some exclude it even for physiological impairment. Verify the benefit before applying medical-necessity criteria.

Tissue and ageThe public policy refers to InterQual. It does not print detailed gram, BMI, age or symptom-duration thresholds to reproduce here.

Documentation and workflowMember-specific plan language governs. Documentation can support criteria without establishing benefits. Obtain the applicable InterQual requirements and product-specific authorization steps from the plan.

Limits to applying this policyA listed procedure code does not establish reimbursement. Do not import another payer’s grams or treatment duration into this document.

Anthem/Elevance · CG-SURG-71Published July 1, 2026; reviewed May 14, 2026. Confirm product adoption.

Symptoms and treatmentQualifying symptoms plus expected tissue removal and a reasonable prognosis of relief. Pain interfering with daily activities requires at least 3 months of adequate conservative care. Alternatives include refractory intertrigo or ulcerated shoulder grooves, or thoracic-outlet symptoms after at least 3 months of adequate conservative care.

Tissue and ageStandard BSA/mass table may be met from at least 1 breast; use the Du Bois formula. Separate large-resection route requires at least 1 kg from each breast plus a qualifying symptom condition. No general BMI ceiling or minimum age is printed in the extracted pathway.

Documentation and workflowHeight, weight, breast size and shape, symptoms and expected tissue removal. Photographs may be requested. Obtain the plan’s authorization instructions separately.

Limits to applying this policyThe restriction concerns liposuction used to perform reduction; the member summary describes liposuction alone. It is not Regence’s separate adjunct exclusion. Fit concerns or posture alone do not establish qualifying symptoms.

Policies checked October 17, 2026. Plans differ; check your own plan. Policy text is not a coverage guarantee.

Start by asking whether your specific product includes symptomatic breast reduction and which policy applies on the proposed service date. Then check every condition in that policy. Pain, rashes or shoulder grooves may support a request, but none alone establishes payment. The cards below keep the major commercial pathways separate. The linked insurance question page gives a focused place to continue the coverage discussion.

Ask your surgeon

“Does my plan include symptomatic breast reduction, and which current policy and documentation requirements will you submit against?”

Insurance and the decision

Does Utah insurance cover breast reduction?

Utah breast reduction coverage depends on the product, and SelectHealth policy 172, reviewed April 16 and revised July 29, 2026, now provides a recovered commercial pathway with independent-practitioner documentation and tissue standards.

Utah products and service dates matter

SelectHealth · 172Implemented April 10, 2002; reviewed April 16, 2026; revised July 29, 2026.

Symptoms and treatmentCommercial/CHIP must meet all criteria. Independent qualified practitioner documents chronic postural backache, chronic neck or upper-back pain, strap grooves or chronic/recurrent intertrigo due to hypertrophy; recommended or provided conservative treatment was unsuccessful. No competing condition is more likely to cause or significantly contribute to symptoms.

Tissue and ageEach breast or the average of both must meet or exceed one of its printed tables. BSA 1.80 lists 441 g per breast; BSA greater than 2.00 lists 600 g. Alternate table only for BMI below 30: below 60 kg (≤132 lb), 225 g; 61–79 kg (133–174 lb), 360 g; above 80 kg (≥175 lb), 575 g per breast. The printed boundary bands are preserved without interpolation.

Documentation and workflowHeight, weight and surgeon’s estimated grams from each breast. No fixed symptom/treatment duration, minimum age or photo mandate printed in this section. Preauthorization responsibility varies by product and provider; confirm it.

Limits to applying this policyMedicare refers to CMS; when no CMS determination and no InterQual criteria are available, commercial policy applies. Community Care typically follows Utah Medicaid including InterQual, with stated possible alternatives. Do not apply commercial thresholds automatically.

Regence / BCBS of Utah · Surgery 60Effective November 1, 2025; reviewed September 2025. Replacement announced November 1, 2026; confirm Utah product and service date.

Symptoms and treatmentAge, tissue and at least 1 clinical route. Pain requires referring-provider records, not the operating surgeon’s, showing at least 12 months, exclusion of other causes and at least 3 months unsuccessful conservative care. Alternatives are intertrigo despite 3 months dermatologic care, x-ray-confirmed kyphosis, or ulnar paresthesia despite 3 months conservative care.

Tissue and ageEach breast meets the printed Mosteller-BSA Schnur minimum, excluding liposuction fat. With qualifying asymmetry, combined tissue must total at least twice the minimum. Age 18; the policy also prints a younger exception for age 16 or older with 12 months stable size and documented necessity before 18. This guide remains adult-only.

Documentation and workflowHeight, weight, laterality and total planned tissue, plus the records supporting the clinical route. Operative records may be reviewed for actual grams. No universal numeric BMI ceiling or photo mandate in the extracted criteria.

Limits to applying this policyAdjunct liposuction is not medically necessary; sole-method liposuction is investigational. The pending replacement was not recovered. Obtain current authorization and appeal instructions rather than applying an old contract.

PEHP · Reduction MammoplastyNo numeric policy ID found. Effective December 18, 2023; next review November 16, 2026.

Symptoms and treatmentAll criteria apply. At least 1 pain/ulnar-nerve condition or inframammary intertrigo, dermatitis, eczema or hidradenitis after at least 1 continuous 3-month appropriate medical-management trial. Other causes must be excluded or breast size documented as exacerbating the underlying condition.

Tissue and ageAge 18 or completed growth. Planned average tissue per breast must be above the printed 22nd-percentile Schnur value. At BSA 1.80 that value is 441 g.

Documentation and workflowOnly some employer groups cover the procedure. Surgery must be preauthorized and reviewed for medical necessity. Payment follows deductible and benefit percentage of maximum allowable fee.

Limits to applying this policyAppearance-only or psychological complaints without objective physical signs are excluded; sole-method liposuction is unproven. No general BMI ceiling or explicit photo requirement is printed in this pathway.

University of Utah Health Plans · §9.182026 Individual EPO Summary Plan Description.

Symptoms and treatmentBreast reduction is listed among exclusions in the cosmetic/reconstructive section. Surrounding reconstructive exceptions require plan interpretation.

Tissue and ageNo general symptomatic-macromastia gram, BMI, treatment-duration or age threshold can be derived from this exclusion.

Documentation and workflowAsk the plan for a written determination for your specific product. The contract describes 1 internal appeal and 1 voluntary independent external appeal; internal filing period is 180 days with online, fax, mail or telephone routes.

Limits to applying this policyThis document does not establish an exclusion across every UUHP product or resolve the interaction with reconstructive exceptions.

Policies checked October 17, 2026. Plans differ; check your own plan. Policy text is not a coverage guarantee.

The SelectHealth commercial pathway is not its Medicare or Community Care pathway. Regence’s opened Surgery 60 document announces a replacement effective November 1, 2026; the criteria shown here describe the version effective November 1, 2025. University of Utah Health Plans evidence is a 2026 individual EPO contract, not every UUHP product. PEHP expressly says only some employer groups cover reduction.

Ask your surgeon

“Which Utah product and service-date policy apply to me, and is any required referring-provider documentation still missing?”

Does Medicare cover breast reduction?

Medicare breast reduction review is local: Noridian LCD L35163, revised effective October 16, 2025 and applicable in Utah, requires at least two listed signs or symptoms lasting at least 6 months despite reasonable nonsurgical care.

Local and regional coverage stays local

Medicare · Noridian L35163JE/JF including Utah; revision effective October 16, 2025; original October 1, 2015.

Symptoms and treatmentHypertrophy plus at least 2 listed findings for at least 6 months despite reasonable nonsurgical care. Findings include attributable upper-back, neck or shoulder pain; attributable headache after other causes are addressed; correlated thoracic kyphosis; chronic weight-related breast pain; refractory inframammary maceration or infection; strap grooves; or upper-extremity paresthesia from weight-related brachial-plexus compression. Evaluation includes competing causes, support garments and refractory skin symptoms as appropriate.

Tissue and ageSurgeon documents expected tissue and rationale. No universal Schnur minimum, numeric BMI ceiling or minimum age is mandated in the opened clinical section.

Documentation and workflowClinical records must also show risk counseling and the proposed tissue amount with rationale. Obtain applicable coding and setting-specific instructions; this LCD is not a complete prior-authorization workflow.

Limits to applying this policyAppearance-driven ptosis or fit concerns are cosmetic. Only covered work is payable when cosmetic procedures are combined.

Medicare · Novitas L35090Revision effective July 11, 2021; original October 1, 2015. Different jurisdiction from Utah’s Noridian.

Symptoms and treatmentSymptoms refractory to nonsurgical measures plus BSA/Schnur relationship. The back-pain route lists analgesia, support and physical therapy. Shoulder grooves must be greater than 1 cm with irritation or darkening.

Tissue and ageThe evidence discussion permits bilateral reduction when at least 1 breast meets the Schnur amount and other criteria are satisfied. Use this LCD’s full logical structure and printed table.

Documentation and workflowNo complete authorization workflow is established by the excerpt. Confirm the responsible contractor and applicable records requirements.

Limits to applying this policyDo not transfer this local tissue rule to Noridian or present it as a national Medicare requirement.

Blue Cross NC · Breast Surgeries, Section IIICommercial regional policy; reviewed August 2026; standalone effective date not stated.

Symptoms and treatmentAll requirements. Significant activity-limiting pain unrelated to other causes or nonseasonal intertrigo; at least 2 examination findings among significant grooves, obvious hypertrophy and suprasternal-to-nipple distance over 28 cm at height at least 5 ft 2 in, or over 25 cm below that height. Pain requires 6 weeks of support bra, NSAIDs if not contraindicated, exercises and heat/cold; intertrigo requires 6 weeks of hygiene, medical treatment and support bra.

Tissue and ageTissue must exceed its BSA table. BMI over 27 triggers documented weight management only for low-back pain/intertrigo with obesity contributing. The alternative documented attempt includes an initial consultation, regular visits over 3 months, diet, aerobic exercise and reasonable compliance.

Documentation and workflowPrior review required. Pictures are unnecessary for documented obvious hypertrophy. Support letters do not replace the specific medical records required.

Limits to applying this policyThis regional Blues policy is not a Utah Regence rule. No general minimum age appears in the extracted symptomatic pathway.

TRICARE · Reduction MammoplastyPublic benefit summary; page date not stated; checked October 17, 2026.

Symptoms and treatmentDocumentation must support medical symptoms such as pain, shoulder grooves or activity limitation.

Tissue and ageNo universal gram, BMI, age or therapy-duration cutoff printed on the public benefit page.

Documentation and workflowPhotos may be requested. Confirm referral/preauthorization with the regional contractor. Medical-necessity appeal instructions differ from factual payment appeals.

Limits to applying this policyThis is a benefit summary with special rules and limits, not individual authorization.

Kaiser Georgia · 01-53 / QRM_1574 / A-0274Historical regional document revised March 14, 2024; next revision March 14, 2025. Current adoption unconfirmed.

Symptoms and treatmentFunctional interference attributable to size with at least 2 listed findings, unsuccessful nonsurgical treatment, no evidence of breast cancer and pictures for review.

Tissue and ageAt least 450 g from each breast and BMI at or below 32 in this dated document.

Documentation and workflowObtain the current member criteria and authorization requirements. This is not verified current eligibility.

Limits to applying this policyNot a national Kaiser cutoff; historical adult pathway only.

Kaiser Colorado · Selected Plastic Surgery ProceduresHistorical section reviewed April 2024; next review April 2025. Section effective date and current adoption unconfirmed.

Symptoms and treatmentSymptomatic pathway requires at least 6 months impairment plus all listed conservative conditions: weight loss, support garments, exercise and/or physical therapy, regular NSAID treatment and the implant-removal condition when applicable. These are policy descriptions, not instructions to take medicines.

Tissue and ageThe document says “at least 400gm or meets Schnur criteria for patients under BSA 1.91.” Its breast denominator and BSA qualifier remain ambiguous.

Documentation and workflowCosmetic implants must be removed at the patient’s expense before glandular reduction under the dated pathway. Obtain current criteria rather than resolving the ambiguity yourself.

Limits to applying this policyNo current national Kaiser rule follows from this regional file. Separate pregnancy-gigantomastia pathway not expanded here.

Medicaid · current-product reviewUtah current criteria behind Utah-ID; NC 1A-12 amended August 15, 2023; Humana Kentucky HUM-KY-2030-001 effective June 24, 2026.

Symptoms and treatmentUtah’s public InterQual 2011 sheet cannot establish current criteria. Current NC and Humana Kentucky documents address specified reconstructive/symmetry indications, not a national symptomatic-macromastia entitlement.

Tissue and ageDo not reuse the old Utah 500 g or BMI 30 thresholds as current criteria. NC’s under-21 EPSDT provisions may affect adults 18–20 and require individualized review.

Documentation and workflowNC and Humana Kentucky require prior approval for the specified procedures. Ask your own Medicaid product for its current criteria.

Limits to applying this policyOther-state reconstructive pathways do not establish Utah benefits or a general national Humana pathway.

Policies checked October 17, 2026. Plans differ; check your own plan. Policy text is not a coverage guarantee.

Noridian asks the surgeon to document anticipated tissue removal and the rationale; its opened clinical section does not impose a universal Schnur minimum. Novitas L35090 uses a different local pathway involving symptoms and body-surface-area tissue weight. These are contractor policies, not interchangeable national gram rules. When cosmetic work accompanies covered care, Noridian limits benefits to the covered procedure.

Ask your surgeon

“Which Medicare contractor or regional plan reviews my operation, and what clinical and tissue documentation does that policy require?”

Insurance and the decision

How many grams does breast reduction insurance require?

Breast reduction insurance has no single gram minimum: Aetna’s table lists 655 g per breast at BSA 1.80, while PEHP’s table lists 441 g at its 22nd percentile and requires planned average tissue above that value in policies checked October 17, 2026.

How many grams does breast reduction insurance require

Symptoms and treatment

Document duration, functional effects, evaluation and the treatments your actual policy requires.

Evidence packet

Gather clinician records, photographs where requested, height, weight and the surgeon’s letter and estimated tissue amounts.

Plan-specific grams

Keep the formula, printed table, comparison operator and breast denominator together.

Authorization

Confirm the benefit and authorization requirements for this product, provider and service date.

Body surface area, or BSA, uses height and weight, but the formula, table and comparison operator belong to the individual policy. Aetna uses Mosteller; Anthem instructs Du Bois. Aetna specifies each breast and excludes fatty tissue from its table requirement. Cigna uses average grams per breast, Anthem permits its standard table amount from at least one breast, and SelectHealth permits each breast or the average. Do not substitute a generic calculator for those rules.

Ask your surgeon

“Which exact table, formula and each-breast or average rule will my insurer use, and how will estimated and actual grams be documented?”

Insurance and the decision

How do I appeal a breast reduction insurance denial?

A breast reduction denial can be challenged through the plan’s appeal process, and HealthCare.gov guidance checked October 17, 2026 distinguishes internal insurer appeals from independent external review.

Keep the appeal route with the plan

Aetna · member appeal instructions

General filing period 180 days after denial unless the plan allows longer. Phone or mail using member-card or EOB information, with comments, records and supporting information.

Cigna · May 2024 form and member guidance

Start with customer service. Usual filing period 180 days, with longer plan periods possible. Submit the form or letter and clinician records for medical necessity; employer-plan process governs. External review may follow internal review for eligible medical-judgment disputes.

SelectHealth · undated member appeal page

Public page states 60 days after adverse determination. Online route or appeal form and appeals-department help are available. Confirm product and denial-notice instructions.

PEHP · Appeal Filing Form

Member or provider may appeal within 180 days of initial determination. A member or authorized representative can provide supporting information; relevant claim-denial information is free on written request. External review is described after appeal denial involving medical judgment.

TRICARE · updated October 5, 2026

Medical-necessity preauthorization appeal uses a letter to the regional contractor postmarked within 90 days, with supporting records. Factual payment appeals differ. Where Medicare and TRICARE both cover the service, TRICARE directs the appeal to Medicare first.

UUHP · 2026 Individual EPO

Internal appeal within 180 days; online, fax, mail or telephone. The contract describes 1 internal and 1 voluntary independent external appeal.

Anthem and UnitedHealthcare · product routes

Anthem California’s member page allows oral/written appeals, an authorized representative and 180 days; that deadline is not universal across Elevance. UHC’s forms page excludes several product groups; choose the form matching your coverage.

Regence · get the current contract

A 2021 Utah Silver EPO contract used 180-day filing periods and a 14-day pre-service response. A 2024 contract gives a 15-day response. Neither establishes the 2026 response period. Filing and decision clocks are separate.

HealthCare.gov distinguishes internal appeal and external review. Follow your denial notice and current member contract; an excluded benefit and a medical-necessity denial are different disputes.

Read the denial reason before responding. A benefit exclusion, missing records, failure to meet a clinical criterion and a tissue-estimate disagreement need different answers. Ask the surgeon’s office which records address the stated reason and who will submit them. In a historical authorization review, 83 of 295 requests were denied, only 18 denials were appealed and 13 appeals succeeded. That selected experience is not your probability of approval.

Ask your surgeon

“If my breast reduction request is denied, what is the reason, who prepares the appeal and which deadline applies?”

Plan your consultation

Can you breastfeed after breast reduction?

Breastfeeding after breast reduction may be possible without a full milk supply, according to CDC guidance checked October 17, 2026, and the 2017 systematic review found better study outcomes when tissue connecting the nipple to the chest wall was preserved.

Can you breastfeed after breast reduction

Before surgery

Discuss future pregnancy, desired feeding options, pedicle design and free-graft possibility.

After childbirth

Tell the maternity and lactation teams about the reduction and the technique if known.

Assess the baby’s intake

CDC recommends close growth monitoring and help with milk supply; making some milk is not the same as meeting every feeding need.

That review included 51 worldwide observational studies and 31 techniques. It deliberately did not statistically pool results because definitions and methods varied. Its main denominator was women who had children after reduction, with separate analyses among those who attempted breastfeeding. Study-level medians must not become a personal probability of exclusive breastfeeding. The operation’s name or outside scar cannot establish how many ducts and nerves remain functional.

Ask your surgeon

“How could my proposed reduction affect milk production, and what lactation support would you recommend if I later have a baby?”

Plan your consultation

Will nipple sensation return after breast reduction?

Nipple sensation after breast reduction can change permanently, according to Cleveland Clinic and Johns Hopkins guidance checked October 17, 2026, so early numbness cannot establish your final sensory outcome.

Will nipple sensation return after breast reduction

Different endpoints

Some touch, normal temperature sensitivity and the same intimate sensation are not equivalent.

Different techniques

Ask how the proposed nipple attachment and possible graft affect the discussion.

Different time points

Discuss early symptoms and follow-up with the team. A single postoperative day cannot predict the final result.

Ask about breast skin, nipple touch, temperature and intimate sensation rather than treating all feeling as one endpoint. ASPS distinguishes a nipple moved on attached blood and nerve supply from a free nipple graft with sensory loss among the trade-offs. Neither an attached pedicle nor a reassuring early response can establish unchanged sensation later. Tell the surgeon what sensation means to you before agreeing to a technique.

Ask your surgeon

“How could this technique change touch and intimate sensation, and when would you reassess a change?”

Plan your consultation

Do I need a mammogram before breast reduction?

Breast reduction preparation may include mammography based on age and personal or family history, according to ASPS guidance checked October 17, 2026; its 2022 guideline also recommends pathology evaluation of removed tissue.

Do I need a mammogram before breast reduction

Before

Review history and images. Resolve the required screening or diagnostic evaluation with the team.

During and after

ASPS recommends pathology evaluation of removed tissue. Ask who communicates the result.

Later

Continue indicated screening and disclose reduction history so postoperative changes can be interpreted.

Bring previous mammograms, breast biopsies and family cancer history to the consultation. Screening, investigation of a new symptom and a payer’s preoperative requirement are different tasks. The USPSTF’s 2024 recommendation is biennial mammography at ages 40–74 for its average-risk population. Aetna separately requires a negative mammogram within 2 years for women 50 or older. Neither statement substitutes for evaluation of your particular history or a new finding.

Ask your surgeon

“What imaging do I need before breast reduction, and how will pathology results and future screening be handled?”

Plan your consultation

What are the risks of breast reduction surgery?

Breast reduction had 6.4% overall complications, 1.8% reoperation and 1.1% readmission in a 2026 NSQIP analysis of 45,143 adult women; these are short-term registry outcomes rather than lifetime risks.

What are the risks of breast reduction surgery

Short-term registry

45,143 adult women in the 2026 analysis. Overall complications, reoperation and readmission are distinct outcomes.

Wound and nipple risks

Discuss delayed healing, fat necrosis and skin or nipple-areola loss separately. Ask how the planned tissue removal affects management.

Urgent changes

Contact the surgical team immediately for the Cleveland Clinic warning signs above. Use your team’s written after-hours and emergency instructions.

ASPS also lists wound-healing problems, fat necrosis, tissue loss at incision junctions, partial or total nipple-areola loss, sensory changes, breastfeeding loss, clots and cardiac or lung complications. Its patient page supplies no individual probability for these risks. Ask which events the surgeon includes in a complication rate, how long patients are followed and how events treated in the office are counted.

Ask your surgeon

“Which complications are most relevant to my operation, and how would your team recognize and manage them?”

Plan your consultation

How do I choose a breast reduction surgeon?

Choose a breast reduction surgeon whose qualifications and experience match the operation you need, and ASPS’s surgeon checklist checked October 17, 2026 specifically distinguishes American Board of Plastic Surgery certification from cosmetic-surgery board labels.

How do I choose a breast reduction surgeon

Credentials and setting

Name the specialty board, relevant experience, operating facility and follow-up team.

Operation and trade-offs

Record scar pattern, nipple attachment, additional procedures, breastfeeding and sensation discussion.

Coverage and practical plan

Record the applicable policy, authorization status, financial responsibility and recovery support.

Ask which board is named, what experience the surgeon has with your proposed reduction and where the operation will take place. ASPS describes accredited outpatient, ambulatory-center or hospital settings. A credential, attractive photograph or social-media following cannot establish your result. Ask who will see you afterward, who answers out of hours and how the practice manages a wound problem, unexpected pathology or another operation.

Ask your surgeon

“Can you explain your credentials, show comparable breast reduction cases and put my surgical, coverage and follow-up plan in writing?”

How we researched this guide

This adult guide uses corrected clinical claims and payer documents checked October 17, 2026. Patient-source questions about pain, insurance, size goals, grafts, breastfeeding and sensation shaped the chapters. Safety and pathology are additional clinical planning topics.

Clinical guidance and coverage differ

A society’s recommendation is not an insurer’s eligibility rule. Policy dates, product limits, each-breast versus average requirements and AND/OR pathways stay with their payer. Plans differ; check your own plan. Policy text is not a coverage guarantee.

Keep study units separate

Patients, breasts, questionnaire respondents and literature studies are different denominators. Registry windows do not measure lifetime risk. Observational findings cannot prove a technique causes a better result; overlapping cohorts are not added together.

Patient research has limits

The supplied voice packet has incomplete original-post and comment review. Sources identify questions, not the frequency of experiences. Institutional explanations are not a representative patient cohort. No forum quotations or prevalence claims are used.

Media and remaining decisions

Only selected videos with Mux playback enter the page and its source list. Research screening is not full-speed audiovisual, clean-frame or rights clearance. Detailed prices and postoperative instructions belong to the cost and recovery guides; specialist feeding and insurance questions have linked pages.
